On 4/23/90, while the plant was at 87% Power, Train B Safeguards i Protection System Safety In ection System (SIS) Go Testing was in l

progress. This test veri'ies the actuation of several SIS components in response to a safety injection signal. In order to conduct this test during power operation, the actuation of certain i

affected SIS components are temporarily blocked, per procedure.

During this test, an operator unblocked the operation of charging Containment Isolation Valve (2CHS-MOV310) before the actuation signal was reset. This caused the valve to close, causing the unit to lose charging to the Reactor Coolant System. Operators reset the signal and restored charging. A Human W Fformance r

these actions. There were no safety implications due to this l event. This event is bounded by the Analysis of Beaver Valley Unit 2 UFSAR Section 15.5.1, " Inadvertent Operation of Emergency Core Cooling System During Power Operation."

Descrintion of Event on 4/23/90, while the plant was at 87% Power, the Train B Safeguards Protection System Safety Injection System (SIS) Go Test was performed. This test actuates individual Safeguards output relays and verifies the operation of required SIS components. SIS components that cannot be tested in this manner during power operation are temporarily blocked from actuating when other components that are controlled by the same Safeguards relay are tested.

The procedure had operators actuate Safeguards relay K603B. This relay will send actuation signals to several charging and safety injection system valves and trip signals to the Train B Containment Air Recirculation Fans.- One of the valves that receive a signal from this relay is [2CHS-MOV310). This valve is a containment isolation valve in the normal charging flowpath to ,

the Reactor Coolant System. In order to prevent the closure of (2CHS-MOV310) during power operation, its breaker is opened before the relay is actuated. The procedure has the operators verify the breaker's contactor actuates in response to the SIS signal. After the SIS signal and the breaker contactor are reset, the procedure has operator reclose the valve's breaker. During the performance of this test, the operators reclosed the breaker for l2CHS-MOV310) before the SIS signal was reset. This caused the va: ve to close, .

Previous Similar Events There was one previous event (LER 88-016) involving an inadvertent ESF actuation duri.ng Go testing when an operator did not properly block a required component.
