j. On 11/6/89, the Unit 2 Train A Service Water Pump seal water -i f
supply automatically transferred to its backup supply due to low I seal water pressure. Pump operation was unaffected. The low pressure occurred when the seal water normal supply (Filtered Water) was inadvertently isolated while de-energizing a Unit 1
Motor Control Center (MCC) and its associated AC Distribution l . Panel for maintenance. Prior to de-energizing the MCC, the Unit 1 Operating Manual Electrical Load lists was used to identify the components tisat would be affected by this evolution. The Unit l'120 Volt AC operating manual failed to list all the valves that were powered from the involved AC Distribution
l. Panel. Due to this discrepancy, the Filtered Water system .

supply valves were inadvertently de-energized, isolating the Filtered Water system. Operators realigned the Filtered Water -

at:pply and restored the Unit 2 Seal Water to its normal arrangement. The Unit 1 120 Volt AC operating manual power i supply list has been revised to include the involved valves.

There were no safety implications due to this event. The Service Water Pump seal water supplies are described in Unit 2 UFSAR section 9.2.1.1.2.

Descriotion of Event At 2020 Hours on November 6, 1989, the Service Water Pump Seal Water low pressure alarm was received in the Unit 2 Control Room.

Operators verified that the service water pump seal water supply had automatically transferred from its normal supply (Filtered Water) to its backup supply (discharge of the service water pump). Service Water Pump operation was unaffected by this transfer.

Investigation determined that the low seal water pressure was a ,

result of the Filtered Water system being inadvertently isolated when the non-safety related 480 Volt Motor Control Center (MCC) 1-1 was de-energized for maintenance. Before de-energizing the MCC, the Unit 1 480 Volt System Operating Manual was used to identify all components that would be affected by this evolution.

This manual showed that MCC 1-1 supplied power to AC Distribution Panel PNL-AC-1. The Unit 1 120 Volt AC System Operating Manual was then used to identify all components energized from PNL-AC-1.

This manual listed Clarifier to Filtered Water supply valve TV-WT-102 as being supplied from PNL-AC-1. Based on this, it was verified that a supply path to Filtered Water would still exist via the Filtered Water Storage Tank to Filtered Water supply valve LCV-WT-102. However, the Operating Manual did not identify that LCV-WT-102 was also powered from PNL-AC-1. This resulted in both TV-WT-102 and LCV-WT-102 being de-energized when MCC 1-1 was removed from service. This isolated both supply paths to the Filtered Water system, causing the system to de-pressurize.

Cause of Event This event was a result of a deficiency in the Unit 1 120 Volt AC System Operating Manual. The manual failed to list all loads supplied by PNL-AC-1. A review of the electrical wiring drawings verified that LCV-WT-102 was powered from PNL-AC-1, although the operating manual did not list it as one of the panel's loads.

The deficiency was the result of basing the load list on the system one-line drawings that did not show multiple loads off a single breaker. When multiple loads were powered from a single breaker, only one load was shown on the drawing.

corrective Actions operators verified that the Service Water Pump was operating properly.and was unaffected by the seal water transfer. Operators opened manual valve WT-97, which bypassed LCV-WT-102 and repressurized the Filtered Water system. The Pnit 2 Service Water Pump seal supply system was restored to its pre-event configuration.

The Unit'l 120 Volt AC System Operating Manual has been revised.

All 120 Volt AC power supply lists were reviewed and updated to include all comp-onents energized from 120 Volt AC power. .
