On 1/24/90, an internal Quality Assurance audit discovered that the Control Rod positions were not recorded ~on the operators logs on 4/25/89 at 2000 Hours. These-readings are required to be recorded, as per Technical Specifications 3.1.3.1 and 3.1.3.2.

.every four hours when the Control Rod Insertion Limit-monitor and Deviation alarm are out of service. This audit found no other omissions of this type. An independent examination of a sampling of operator logs found no similar-errors. The involved personnel were counseled concerning this event. There were no safety implications due to this event. The operator logs for 4/25/89 at 1600 Ilours and 4/26/89 at 0000 Hours showed that the control rods '

were. aligned to within 5 steps, while the maximum allowed deviation is 12 steps.

0l0 0l2 0F 0l2 Descrintion of Event In January 1990, the Beaver Valley Quality Assurance (QA).

Department performed an internal audit of the Operations group's compliance with Technical Specification requirements. On January 24, 1990, this audit discovered that the Operation Surveillance Verification Log-LS-3 was not completed-for April 25, 1989. This log is used to record the control rod analog position indications .

This violated the surveillance requirements of Technical Specifications 3.1.3.1 (Reactivity control Systems - Movable Control Assemblies) and 3.1.3.2 (Reactivity Control Systems - Position Indication Systems).

Cause of Event The QA audit-found no other errors of this type. The Operations department conducted an independent check of a sample of completed Operation Surveillance Logs and found no other problems of this type. Based on these examinations, it appears that this event was an isolated case, f Corrective Actions l
