IV. Event Descripticn: ,

On December 19, 1982, at 1500 hrs. during Reactor Startup, the Unit NSO observed that IRM "B" was reading'downscale. Upon investigation by operations, it was discovered that the High Voltage cable leading to IRM "B" had been removed. A Caution Card, written by the Instrument Maintenance Department, was found attached to the cable stating "Use Caution when removing High Voltage Cable - Center Post of plug is broken - New one on order Work Request #21028 dated 12-13-82" IRM "B" had previously been declared Inoperative on 12-10-82 per Degraded Equipment Log entry.

V. Probable Consequences:

At the time of the occurrence, LaSalle Unit I was critical in the Startup Mode. The remaining 7 IRM's were operable. Technical Specification of Table 3.3.1-1 requiring 3 operable IRM channels per RPS Trip system was not violated. Safe operation of the plant was maintained at all times.

. VI. Cause:

The high voltage cable leading to the IRM #B" ' detector was last verified to be connected on December 13, 1982 when the Instrument Maintenance Department attached a Caution Card to the cable after troubleshooting IRM "B" according to LAP 1300-1 for Work Request

  1. L21028 It could not be determined for what reason the High Voltage Cable leading to the IRM "B" detector had been disconnected.

Since the installation of the High Voltage cable leading to the IRM "B" detector is not required for the Instrument Maintenance Department

! to satisfactorily complete LIS-NR-02, IRM Rod Block and SCRAM Functional Test, and since a cold startup was performed (i.e.:

IRM's were not on range till after the Reactor was critical), this

! condition was not previously observed.

{ VII. Corrective Action: ,

l Upon discovering the High Voltage cable leading to IRM "B" detector -

disconnected operations ininediately suspended startup. The High Voltage cable was reterminated and IRM "B" was placed in BYPASS.

A review panel was convened consisting of the Plant Superintendent, Operating Assistant Superintendent, Operating Engineer, Technical Staff Supervisor and Instrument Maintenance Supervisor to discuss the events leading to this report.

It was concluded that training will be conducted with the Operating '

Department on the use and control of the Degraded Equipment Log and the requirements to properly identify degraded equipment.
