On December 21,1982 at 0300 hours0.00347 days <br />0.0833 hours <br />4.960317e-4 weeks <br />1.1415e-4 months <br /> during the performance of LOS-PC-Q1, Primary Containment Isolation Valves Operability Test and Inservice Inspection for Conditions 1, 2, and 3, Reactor Recirculation Coolant Sample Outboard Isolation Valve, IB33-F020, would not close when demanded from the Control Room.

V. PROBABLE CONSEQUENCES OF THE OCCURRENCE:

At the time of the occurrence the reactor was in the RUN mode at 24% MWT, 170 MWE.

The Reactor Recire. Coolant Sample Inboard Isolation Valve, 1B33-F019, was deenergized in the closed position per Technical Specification 3.6.3.

Safe plant operation was maintained at all times.

VI. CAUSE:

The E.T.O. Air Supply Solenoid Valve for 1B33-F020 was found to be sticking in the open position. This caused the air operator of 1B33-F020 to keep the valve open. 1B33-F020 is a 3/4" valve, type 70-18-9 DRT, manufactured by WKM valve division of ACF Industries.

VIl. CORRECTIVE ACTION:

Work Request #L21242 was written on 12/21/82 to troubleshoot the 1B33-F020 valve. When maintenance personnel attempted to recreate the problem, the valve cycled properly. This work request was then closed out on 12/23/82.

The Process Sample for the Reactor Recirculation System was then returned to service.

The 1B33-F020 valve failed to cycle properly again on 1/2/83 and Work Request #L21436 was written to troubitehoot. The reactor was in Opera-tional Condition #4 at the time, therefore, the affected penetration was not isolated per Tech. Spec. 3.6.3. The soledoid valve for 1B33-F020 was disassembled and cleaned and then reassembled. Work Request L21436 was completed on 1/7/83.
