ESFAS A lonic seauencer failed l 1013ll renderine #12 Diesel Generator (D/C) out of service to U/2 (T.S.3.8.1.1) . l 10l4ll The remaininn A.C. electrical power sources were verified to be operable I lo 15 l l at 1121. The secuencer was replaced restorinn #12 D/G to service to U/2 l 10l8ll at 1350. The ensuing investigation revealed the failure would not have l lol7ll prohibited the sequencer from completing its safety function, therefore l lol8 l #12 D/G was never inoperable. Similar event: 50-317/82-85. l

4. 4 44 4, CAUSE DESCRIPTION AND CORRECTIVE ACTIONS h l110ll Insnection of the secuencer module (Vitro 1628-1076) by the vendor re- I l1l1ll vealed the failure was caused by intermittent operation of the module's l l1l2ll test push button. The part was replaced and the module was returned to l ltl3ll stock. This failure would not prohibit the seauencer from performine its l 1 4 l intended desinn function. No further action is deemed necessary. I
