U INTERIM REPORT FOR PRD-80/03

1. Description of Deficiency:

Some 132 cables located under the Unit 1 Reactor Pressure Vessel were damaged during the installation of control rod drive assembly housings. The damage ranges from pinched insulation to insulation cut through to the conductor. This is documented in several Bechtel Nonconformance Reports (NCR) and Condition Reports (CR) and filed under Bechtel MCAR-GGNS No. 62.

The investigation as to the cause of this deficiency is continuing.

II. Safety Implications Some of the damaged instrumentation cables are associated wi:h safety related equipment, and as such, could have adversely affected the safety of plant operations at Unit 1.

III. Corrective Action Taken:

The NCR's and CR's mentioned in Item I above, were dispositioned to document the deficiencies and to accomplish the required corrective actions. The cables in question will be repaired or replaced, as required. These corrective actions are presently in progress. Revisions to the procedural and administrative controls pertaining to the installation of the housings are under development. This latter issue will be addressed in the final report.
