On 13 Dacer.ber 1974, Reactor Gul'iding Energency Cooler B Catlot Valve R3-V4B failed to open while parfaming the En 3ineered Safeguards Ranctor Building Isolation crid Ccoling Quarterly Surveillence Test.

Tira redundant Erm.,2ncy Cooler Outict Valves RR-V4A, 4C, end 4D were tested satisfccterily.

An initial investigation indicated the RR-V43 coaler outlet valve failed to o*,.:n d:m to a malfunction of the valve motor operator torque switeh.

The terque switch will be repaired or replaced and the valve will be appropri-atoly tasted.
