Type:

An abncrmal cccurrence as defined by the Technical Specifications, paragraph 1.3g, in that these two isclaticn valves being ir the incorrect pcsition constituted an inadecuacy in the implementation of procedural centrols that threatened to cause an unsafe condition in the cperation of the plant.

(6) Description of Occurrence: Cn September 18, 1975, at 0350 hcurs, while performing a Reactcr Building Personnc2 Access Eatch Docr Seal Leak Test (SP 1303-11.18 Section A), the operator checked the valve lineup for the air supply to the decr seals. He fcund that tvc normally cpen valves (PP-V kT and 179) were in the clcsed pcsition. These valves were opened immediately and the correct pcsitiens of other sbnilar valves in the E=ergency Access Hatch were verified. A leak test was performed and the results were satisfactory. (7) Designation of Apparent Cause of Occurrence: The apparent cause cf this cccurrence was perscnnel in that the sur elllance procedure (SP 1303-11.15) required that the subject valves be re-opened after any testing which would require their cicsure and the precedure was apparently not folicved. (8) Analysis of Occurrence: It is considered probable that PP-V kT was c1csed since September 10, 1975, when the recorded data began to consistently indicate no hatch leakage. The closure of PP-V-kT would have prevented autenatic pressuricaticn of t'e door seals if an Energency Safeguards actuation veuld have cccurred. If PP-V-179 had been cicsed during a period when PP-V kT was still open, any decr seal tests veuld have verified leak tightness of the inner hatch decr only. There is no way cf verifying the pcsiticn of PP-7-179 during this time pericd therefcre, it must be assumed that all leak tests en the Reacter Euilding Personnel Access Hatch Docr Seals, during the time when PP-7 kT was closed, were invalid because the test air supply could not reach either the inner er cuter decr seals. It has been determined that there was no threat to the health and safety of the public in that: a. the leak rate test performed fc11cving disecvery of this improper valve lineup was satisfactcry and was perfer ed with no adjusteents cr repairs pricr to cr folleving testing, indicating good seal condition and, b. although, the decr seal interspace cculd nct be pressurized, due to this improper valve lineup, leakage past the decr seals in the event a LOCA had cccurred vculd have been negligible, since both decr seals were in good condition as indicated by testing. }476 293 (9) corrective Actien: In addition to tha 4--adiace corrective actions listed above long tern corrective actions will be or have been taken as indicated belev: a. Initiated Tempcrary Preced"*a F"ange to require verifying the pcsition of the valves in air lines to the decr seals prior to each leak test. b. Initiated verk request to install locking devices on the valves inside the perscnnel and emergency access hatch, c. Will place applicable valves en the plant locked valves list when the locking devices are installed, and will then cancel the tenpcrary require =ent to verify valve pcsitions prior to each leak test. The Plant Crerations Review Ccmaittee and Station Superintendent have reviewed and approved the above corrective acticns and have taken steps to assure ecmpleticn of the yet to be ccepleted long ter ccrrective actions.
