This telegram is to confirm the conversation between. Lee Spessard (Region I-NRC). ana J.J. Colitz (TMI Unit 1 Superintendent) at 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br /> 17 June 1975.
The following report is submitted in accordance with paragraph 6./.2 a.5 (page 6-11) of the technical specifications.
Following motor shaft failure described in A0 75-14 the motor for Decay 1! eat River Water Pump DR-P-1B was repaired at the vendors repair facility (Westinghouse - Philadelphia). The motor was returned to Three Hile Island, installed in the system and placed in operation without appropriate quality control cocisnentation.
The installation and use of the motor without proper documentation as to repairs actually perfonned constitutes an inadequacy in the implecentation of QA Plan administrative control that could have caused an unsafe in the operation.of the plant.
The events leading to the violation of the quality control procedures are being investigated and corrective action will be taken to preclude such a
inadequacies in the future.
