APR016 1.0 PURPOSE The purpose of this procedure is to partially implement Section 6.8, "Recovery Termination and Reporting Requirements," of the SHNPP Emergency Plan, which is a regulatory commitment.

This procedure discusses the record keeping and documentation requirements for emergency preparedness.

TI-906, "Training Records" 3 ' RESPONSIBILITIES 3.1 Record Pre arers Functional team leaders, emergency communicators, phone talkers, log keepers, supervisors, managers and directors are responsible for assuring that legible records (checklists, surveys, forms, messages, and logs) of completed actions are prepared and maintained during an emergency. Personnel are responsible for supplying these records to their director or manager.

3.2 Site Emer enc Coordinator, Emer enc Res onse Mana er, Directors , and Mana ers The Site Emergency Coordinator, Emergency Response Manager, direc-tors and managers are responsible for seeing that their subordinates maintain appropriate records, for collecting these records, and for supplying these records to the Senior Specialist - Emergency Pre-paredness when the emergency is terminated.

APR616 3.3 Senior -S ecialist - Emer enc Pre~aredness The Senior Specialist - Emergency Preparedness is responsible for collecting and maintaining all emergency related records following an emergency. He is also responsible for maintaining all emergency preparedness records not specifically assigned to another group.

4. 0 DEFINITIONS None Applicable 5.0 GENERAL Corrections to zecords shall be made by drawing a single line through and initialing the inc'orrect entry, and not by erasures oz the use of correction fluid.

1. Records of emergency preparedness tzaining are maintained by the Harris Training Unit in accordance with TI-906.

9.2 Drill Records

1. Records of walkthroughs performed as part of initial training are maintained as part of the initial training record by the Harris Tra'ning Unit (See PEP-403 and TI-906).

l APR016 kllh a 9.3 Exercise Records Normal recozds (required by plant procedures) and emergen-cy records (required by emergency procedures) vill be prepared by appropriate personnel (managers, log keepers, etc.) as required during exercises (See PEP-406).

2; The Senior'pecialist - Emergency Preparedness vill collect and review exercise records and will maintain any considered to be necessary for reference or demonstrating compliance vith regulations.

9.4 Audit Records The Corporate Quality Assurance Department vill perform an independent audit of the SHNPP Emergency Preparedness Program on an annual basis. The zecords of these audits vill be stored by the gA department as per their proce-dures with a copy to the Senior Specialist - Emergency Prepazedness and othez groups as specified in Section 5.1.6 of the SUPP Emergency Plan.

2. The Senior Specialist - Emergency Pzeparedness (or others if appropriate) will prepare a response to any findings as necessary for the Plant General Manager, and a copy of the response vill be maintained with the copy of the findings for at least 3 years.

9.5 Facilit and Equi ment Records Records of facility and equipment inventories as specified in PEP-402, vill be maintained by the Senior Specialist-Emergency Preparedness for a minimum of three (3) years.

9.6 Emer enc Recozds Team members, emergency co~unicatozs, phone talkers, log keepers, and other personnel vho fill out surveys, ra-diation vork permits, dosimetry records, or other foz=s as a necessary part of their emergency function vill prepaze such records in a legible, accuzate and timely manner.

Such records are forms included in or referenced by Plant Emergency Procedures.

2. Log keepers will prepare logs of events, messages, and major decisions in accordance vith PEP-405 for the manager or director for vhom they are keeping the log.
3. Directors, managers, and supervisors vho do not have a log keeper should keep a log of major events, messages, and major decisions in accordance with PEP-405.

1. 0 PURPOSE The purpose of th s procedure is to implement Sections 2.3 "Command and Control" and 2.4.2 "Site Emergency Coordinator" of the SHNPP Emergency Plan while the emergency is managed fraa the Control Roaa, which is a regulatory conaaitment. It also impleaents corrective action in response to NRC Emergency Preparedness Appraisal Inspection Report Number 5~0/85-09. This procedure describes the actions of the Site Emergency Coordinator from the time an ewergency is declared to the time when the emergency is terminated or when the Site Emergency Coordinator function is transferred to the Technical Support Center.

3.0 RESPONSIBILITIES 3.1 Authority and Reoortin Res onsibi'ities Prior to activation of the Emergency Operations Facility, the Site Bmergency Coordinator is delegated the authority to act on behalf of the Company to manage and direct all emergency operations involving SHNPP.

Upon activation of the Emergency Operations Facility, the Site Emergency Coordinator Control Room reports to the Emergency Response Manager.

3.2 hssi t and Relief of Position

1. The primary person assigned to the position of Site Emergency Coordinator Control Room is the Shift Foreean.
2. If the Shift Foreman is unavailable, the designated alternates in order of succession, until transfer of the Site Bmergency Coordinator position to the Technical Support Center, are:
a. Control Room Senior Control Operator
b. Roving Senior Control Operator
3. The Site Emergency Coordinator may be relieved by a designated alternate trained {in accordance with procedure PBP-403) to perform the duties of this position.

a. When contacted by the State, after an initial notification of an emergency, briefing the State on plant conditions and actions being taken.
b. Recommending protective actions which should be taken by

c. Keeping the Corporate Emergency Operations Center Manager (when the CEOC is activated), the Nuclear Regulatory Cosaission, and appropriate State and local agencies informed as to the status of the plant and radiological effects offwite.
d. Providing updates to the State on the emergency condition and recommendations for protective actions at 30 80 minute intervals, or if'ecessary, more frequently.

NOTE: After the Technical Support Center is activated, the function af the Site Emergency Coordinator transfers from the Control Room in the -person of the Site Emergency Coordinator - Control Room (Shift Foreman) to the Technical Support Center in the person of the Site Emergency Coordinator - Technical Support Center (Plant General Manager). The Site Emergency Coordinator - Control Roam (Shift Foreman) may then assume the position of Operations Leader in charge of the Control Room.

3.4 Duties Relin ished on Activation of the Emer en rations

~Fsci1lt NOTE: This section is provided in the event the Emergency Operations Facility is activated prior to activation of the Technical Support Center.

Upon activation af the Emergency Operations Facility (BOF), the Emergency Response Manager is responsible for relieving the Site Emergency Coordinator of. interfacing with the Corporate Emergency Operations Center, Plant bfedia Center and off~ite agencies, and for managing and directing aII off~ite related emergency operations involving the plant. This includes notification, protective action recommendations, dose projection environmental monitor~ and, cantact with off~ite support groups, with the exception of haiediate response groups such as fire departments, reams squads, and local law enforcement agencies.

3.5 Dele tian of huthorit The Site Emergency Coordinator shall not delegate the responsibilities of:

5. 0 GENERAL The Site Emergency Coordinator Control Room exercises overall command and control of on-site activities.

6.0 INITIhTING CONDITIONS

1. The Shift Forcaan has declared an Unusual Event, Alert, Site Emergency or General Emergency.
2. This procedure is terminated under the following conditions:
a. The functions of thc Site Emergency Coordinator Control Room are turned over to the Site Emergency Coordinator-Technical Support Center.
b. The Site Emergency Coordinator Control Rooa declares that the Emergency is terminated.

7.0 PRECAUTIONS AND LIMIThTIONS

1. Planned radiation exposures in excess of regulatory limits are appropriate only for lifesaving activities, or to prevent additional damage to the plant or radiological releases in excess of the protective action guidelines listed in PEP-104.
2. If telephone calls are received from the public or press, do not release any information. Refer thea to Corporate C~ications.

CAUTION The notification in step 12 in .this section aust be initiated within 15 minutes after declaring that an emergency exists.

If Security has not been notified previously according to PEP-101, contact Security and instruct them to imnediately commence preparation of a shelter list.

2. Initiate and maintain an Emergency Log in accordance with PE~05. Record in this log significant decisions or actions.

Designate a Senior Control Operator as Operations Leader and instruct him to implement Abnormal anc}/or Emergency Operating.

Procedures as appropriate to the situation.

Designate an operator as Emergency Communicator - Control Room and direct him to implement PEP-217. I

5. If assistance is required frcaa an iunmhate response organization (Fire, Ambulance, etc.), ensure that the Emergency Camamicator - Control Rooa implements Section 9.1 of PEP-301.

10. If a radiological release. is hnown or suspected to be occurring, or if.such a release"is anticipated to occur based on plant conditions, designate an operator to perform a dose projection in accordance with PEP-343 or PEP-341, in that order of preference.

Determine what protective actions to rec~nd to off~ite authorities. This decision shall not be delegated. If the classification is. General Emergency, sake the decision in accordance with PEP-104. Do not wait for a dose projection it has if not yet been conpleted. If the emergency involves the release of chlorine, deternine a protective action recceaaendat ion in accordance with PEP-385.

12. Ensure that the Emergency Communicator notifies the State and counties in accordance with Section 9.2 of PEP-301. If necessary to accomplish this notification within the required 15 minute time limit, direct the Emergency Camunicator-Control Room to suspend any other notifications that ere in progress, then return to any suspended notifications after the State and County authorities have been notified.

NOTE: If the declared emergency is the result of emergency action level(s) indicating a high classification (e.g., Site Emergency) that after a brief period is downgraded to a lower classification (e.g., Alert), the offmite notification must indicate the period(s) of time that the higher classification existed and the mitigating conditions that caused the emergency classification to be downgraded-If the downgrading of the eeergency class ocmms during an initial notification, immediately change the notification to state the new eeergency class.

9. 2 Personnel hccountabi lit CAUTION The steps in this section must be ccepleted within 30 ainutes afte declaring the eaergency.

If the classification is Site Eaergency or General Eaergeacy, perforN personnel accountability in accordance with PEP-382.

2- If the classification is Unusual Event or Alert, perform accountability if plant or external conditions are such that injuries to personnel aay have occurred or as be occurring.

3. Ensure that accountability for persons assigned to shelters 3A and 3C is performed in accordance with PEP-382, and that PEP-215 is iaplenented for those shelters.

If any personnel are repor.ed missing after accountability for the site is complete, direct the First A'd Team leader to implement search and rescue procedures in accordance with PHP-394.

9.3 hdditioaal Notifications ChUTION The notification in Section 9.3.1 aust be completed within one hour after declaring the emergency.

Hnsure that the Emergency Coaaunicator notifies the Nuclear Regulatory Ccanaission in accordance with Section 9.3 of PEP-301.

9.4 Follow-u hctions I

NOTE: The 'fol1owing steps may be performed in any order (or more than once) as deemed appropriate.

9.4.1 Notific tions If the eaergency classification is Alert or higher, ensure that the Emergency Coaaunicator notifies the Institute of Nuclear Power Operations in accordance with Section 9.4 of E'HP-301.

l. If you are relieving another individual, reviev the activity log and obtain a briefing on the emergency and any actions that have been completed or are in progress.
2. Report your arrival in the Technical Support Center to the Site Emergency Coordinator and inform him that you are assuming (or relieving if appropriate) the position of Logistics Support Director. Sign in on the chart in the back of the Site Emergency Coordinator's Staff Room.
3. Conduct all communications in accordance vith PEP-302.
4. Initiate and maintaip a record of your activities in accordance Mth PEP-405.

8. Verify that the Emergency Security Team leader is implementing PEP-384 Access Control, SP-015, "Emergency Plan Support" and PEP-210, "Emergency Security Team Leader."
9. Verify that the Assembly Area Leaders are implementing PEP-215, "Assembly Area Leader."

10. If activation of the Emergency Operations Facility has been directed, ensure that the Emergency Security Team Leader provides access control in accordance with SP-015.

9.2 Accountabilit and Evacuation Actions

l. If the Site Emergency Coordinator had decided that it is necessary to account for on>>site personnel, obtain a list of any unaccounted for personnel from the Emergency Security Team Leader as per PEP-382. Report the name(s) and suspected location(s) of missing personnel to the Site Emergency Coordinator and Plant Operations Director, who will direct the implementation of search and rescue efforts in accordance with PEP-394.

Note: The 30 minute requirement to accomplish personnel accountability is satisfied when the Logistics Support Director is notified of the names of missing persons.

2. If an evacuation is to be performed, notify the~ Emergency Security Team Leader and Assembly Area Leaders, inform them where the Radiological Control Teams will be setting up monitoring and decontamination areas (if this is to be done), and instruct them to direct people to the appropri-ate areas and control personnel there.
3. If a recommendation has been made to the State and Counties to evacuate emergency planning subzone A, implement an "Exclusion Area Boundary Evacuation," per PEP-381 and ensure the following actions are taken:

9.3 Continuin Actions
