OS2 1.0 PURPOSE The Maintenance Environmental Qualification Procedure provides for performing corrective and/or preventive/predictive maintenance on environmentally qualified components. It also provides for a means of identifying and reporting potential EQ deficiencies and a means for tracking EQ task requests/revisions.

2. The Senior Engineer - Maintenance FieLd Support is responsible for the following!

3. The Senior Engineer - Maintenance Staff is responsible for the following:
a. Identification of potential EQ deficiencies.

bi Create task routes.

c. Assign route numbers.
d. Updating EDBS and AMMS with route numbers and schedule dates.
4. The Maintenance Planner/Analyst is responsible for the following:
a. Reviewing EQ Supplement to the Vendor Technical Manual to ensure requirements are identified on WRM, ~

bi Reviewing maintenance procedures listed on EDBS 402 screen and listing applicable procedures on WRhA.

c. Identification of repetitive failures and requesting analysis in accordance with MMM-013.
d. Recognizing Category II equipment on EDBS 402 screen when planning a WRSA.
5. The Maintenance Technician is responsible for identifying potential EQ deficiencies found while performing Corrective and/or Preventative Maintenance and initiating a Maintenance Feedback Report.
6. Maintenance Clerk
a. Maintains EQ traveler log, Attachment 6.4.
b. Attaches traveler to task change request and task approval document.
c. Assigns number to EQ traveler.
d. Files completed travelers'.

Maintenance Foreman

a. Schedules EQ Maintenance to be pe~formed on time.
b. Ensures that Corrective Action on WRA establishes the root cause of EQ equipment failures.

c. Requests failure analysis in accordance with MM}{-013 if root cause of failure is in question.
d. Notifying the Project Engineer - Maintenance Field Support when EQ equipment is refurbished or replaced prior to schedule date so that STSS and AMMS can be rescheduled.
e. Notifies Shift Foreman and Maintenance Field Support Engineering if EQ Maintenance cannot be or is not performed on time.
8. Regulatory Compliance is responsible for scheduling MPT procedures on the Surveillance Test Scheduling System (STSS).

3. Potential EQ Deficiency - a deficiency that challenges the qualification of equipment.

5 ' PROCEDURE S.l Maintenance on E Com onents

1. For all maintenance on EQ components the planner/analyst shall:
a. Review EQ Supplement to Vendor TechnicaL Manual to assure all EQ requirements are identified on WRY.
b. Review EDBS 402 screen for procedures required to maintain EQ integrity.
c. Review PLP-400, Attachment 6.2, Postmaintenance Testing Guide, for any Maintenance PMTR that may be required.
d. Obtain guidance from Maintenance Field Support Engineering if problems are encountered interpreting or evaluating EQ requirements.
e. Identify repetitive failures and requests analysis in accordance with MMM"013.
f. h~tify Maintenance Field Support Engineering if Category II equipment needs to be replaced. Place WRM on hold pending resolution by Maintenance Field Support Engineer.

2. Maintenance Field Support Engineering shaLL:

ai Provide support to the planner/analyst, if required, in interpreting or evaluating EQ requirements.

b. Report potential EQ deficiencies to the Shift Foreman by initiating a SOOR in accordance vith AP-615 Verify that EQ requirements resulting from changes in the vendor manuals or EQ Supplement changes are identified and scheduled upon notification of such changes',

Prepare JCO in accordance with AP"618, if required.

e. Request an Engineering Evaluation in accordance vith MOD"103 to establish a "Sound Reason to the Contrary" if Category IZ equipment needs to be replaced.

3~ The Maintenance Technician, while performing either Corrective, Preventive, or Periodic Maintenance, shall;

a. Identify potential EQ deficiencies discovered while performing CM, PM, or MPT/MST by hanging a deficiency tag and initiating a WRY.
b. Annotate potential deficiency in Corrective Action block of WRSAi
c. Inform Shift Foreman immediately and Maintenance Field Support Engineering by Feedback Report of potential deficiency.

5.2 E Task Chan e Re uest Trackin S stem A task change request will be initiated per'PLP-103 any time there is anev task identified or there is a change to an existing task.

1. The initiator of a task change request (PLP-103, Attachment 2) will.provide the task change request to the Clerk who viLL attach the EQ traveler, Attachment 6.1, assign a unique number, and make log entry.
2. The CLerk wiLl initial and date the appropriate block an the traveler, and forward the EQ package to the appropriate Project Engineer - Field Support.
3. The Project Engineer - Field Support vill initial and date traveler, and assign the task change request to the appropriate Field Support Engineer for review.

4. The Field Support Engineer will review the task change request for the following!
a. Equipment is properly identified.
b. Equipment is assigned a start date.
c. Correct frequency has been established.
d. Intent of the EQ requirement is adequately met.

e Evaluate task to ensure that no violation has occurred.

f. Initiate a procedure change request, for a new task or task revision, if required.

5~ The Field Support Engineer will initial and date the traveler and forward the package to Maintenance Staff Engineer.

6. The Maintenance Staff Engineer will perform the following!

ai Create route for the task.

b. Assign route number.
c. Load procedure on EDBS 402 screen and route on EDBS 430 screen.
d. Load route on AMMS.
e. Initial and date traveler and forward package to Maintenance Manager for approval.
7. Maintenance Manager signs and dates task change request and initials and dates traveler and forwards to the Clerk.
8. Clerk initials and dates and forwards package to Regulatory Compliance and annotates log of date completed.
9. Upon receipt of the task change request package, Regulatory Compliance shall perform the follpwing:
a. Assign task number if new task, and update STSS according to the task change request.
b. Print out task approval document.

c. Initial and date traveler.
d. Return task approval document and traveler to Maintenance Clerk.

11. The Project Engineer - Field Support will initial and date traveler and assign the package to the appropriate field support engineer for review.

e. Verifies AMMS and EDBS information loaded by Staff Engineer is correct and verifies STSS Approval Document information loaded by Regulatory Compliance is correct.

14. Maintenance Manager signs and dates task approval document, initials and dates traveler, and forwards to Clerk.
15. Clerk initials and dates traveler and forwards package to Regulatory Compliance.
