Technical Specification Surveillance Requirement (TS) 4.9.2.c requires an analog channel operational test be performed at least once per seven days on each of two Source Range Neutron Flux Monitors (EIIS:IG) while in Mode 6, Refueling. TS 4.0.4 prohibits entry into an operational mode unless the surveillance requirements associated with the Limiting Condition of Operation have been performed within the specified interval. or unless otherwise specified. Maintenance Surveillance Test (MST) -I0169, Nuclear Instrumentation Source Range N31 Operability Test, and MST-I0170, Nuclear Instrumentation Source Range N32 Operability Test, satisfy TS 4.9.2.c.

At 2259 hours0.0261 days <br />0.628 hours <br />0.00374 weeks <br />8.595495e-4 months <br />, the plant entered Mode 6, when the reactor vessel head closure bolts were detensioned. Surveillance tests, MST-I0169 and MST-I0170 had not been performed as required prior to entering Mode 6, violating TS 4.0;4. At 0730 hours0.00845 days <br />0.203 hours <br />0.00121 weeks <br />2.77765e-4 months <br /> on August 15, 1988, Regulatory Compliance personnel were conducting a review of Mode 6 surveillance requirements. It was then discovered that these weekly tests had not been performed and the Maintenance Foreman responsible for performing the test was notified. At 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br />, the Maintenance Foreman and Operations Shift Foreman discussed the requirements for running these surveillance tests and agreed they should have been performed prior to entry into Mode 6. CAUSE Surveillance tests with a frequency of 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or .greater are automatically scheduled on the Surveillance Test Scheduling System. The surveillance procedure schedule is pr'nted for a routine report based on the frequency of the 'test, i.e., if the frequency is weekly, the test will print on the schedule every week. MST-I0169 and MST-I0170 did not schedule automatically because Regulatory Compliance had previously removed 'them from the schedule.

This was due to the tests being a Mode 6 requirement andrit was undesirable to have those tests print on schedule for the long term the plant was not in Mode

6. Since they did not print out automatically, it was not readily apparent that the tests needed to be performed.

ANALYSIS MST-I0050, Calibration of Nuclear Instrumentation System Source Range N31, had been performed satisfactorily in its entirety on August 7, 1988. This test satisfies the requirements of MST-I0169, therefore, source range monitor N31 was in compliance with the seven day requi.rement of TS 4.9.2.c when the plant entered Mode 6 on August 12, 1988. This action statement requires that all operations involving core alterations or positive reactivity changes be immediately suspended when one of the source ~ange monitors is inoperable. The plant was in compliance with this requirement as there were no core alterations or positive reactivity changes in process. No safety consequences resulted'rom this event. This event is reportable in accordance with 10 CFR 50.73(a)(2)(i)(B) as a noncompliance with TS 3.9.2.a and 4.0.4.
