ATTACHMENT 1 REPLY TO NOTICE OF VIOLATION NRC INSPECTION REPORT NOS. 50-250/89-18 and 50-251/89-18 FINDING T.S. 6.8.1 requires written procedures be established., implemented and maintained that meet or exceed the requirements and recommendations of Section 5.3 of ANSI N18.7-1972. Section 5.3 of ANSI N18.7-1972 requires nuclear power plants be operated in accordance with written procedures.-

Procedure 4-0NOP-049, Re-energizing Safeguards Racks After Loss of Single Power Supply, provides instructio'ns for re-energizing safeguards racks without initiating an inadvertent safeguards actuation.

Contrary to the above, on April 12, 1989, without following 4-0NOP-049, a licensed operator installed fuses in a safeguards rack, resulting in re-energization of the rack and an inadvertent actuation of Train A of safeguards.

RESPONSE

1) FPL concurs with the finding.
2) The reason for this event is inadequate administrative controls for this activity. The operator did not recognize the need to use procedure 4-ONOP-049 to install these fuses because the standard clearance for this activity did not specify its use, nor was there any warning specifying its use in the Safeguards racks.
3) a) Warning signs were posted in the Safeguards racks of both unit's.

These signs state that installing fuses FU4 or FU3 may result in a Safety Injection signal being generated, and to use procedure 3/4-ONOP-049 to install either fuse .

b) Warning signs were placed at the power supply breakers to the Safeguards logic for both trains in both units . These signs were placed because the closing of the power supply breaker to the Safeguards logic can also result in a Safety Injection signal being generated .

4) The standard clearances for this activity in the Plant Clearance Order Network (PCON) have been revised . PCON is a computer system used to assist in the initiation, control, and maintenance of clearances .

These standard clearances were revised to include the instruction that procedure 3/4-ONOP-049 be used when releasing these clearances .

5) The corrective actions described above were completed on or before

April 28, 1989.
