Palo Verde Nuclear Generating Station (PVNGS)

Units 1, 2, and 3 Initial Operator and Requalification Retake Examinations File: 90-001-762; 90-A-056-026 As requested in Reference (A), Arizona Public Service (APS) is providing approved reference material for use in administering the initial operator and requalification retake simulator examinations scheduled for March 20 and 21, 1990, at PVNGS. These examinations will'be for one operator who failed the simulator examination during the October-November 1989 requalification examinations administered by the NRC, and an additional operator who was administered an initial examination by the NRC during, August 1989. In the instance of the operator examined during August 1989, the NRC has determined that another simulator examination needs to be administered before a licensing decision regarding his performance on the operating examination can be made.

Based upon a telephone conversation between Mr. M. J. Royack of your staff and Mr. T. C. Cannon, PVNGS Operations Training, it was determined that the reference material transmitted to you by Reference (B) for use in the October 1989 Requalification Program Evaluation would be appropriate for the simulator examinations scheduled for March 20 and 21, 1990, with the following modifications:

'Mr. John B. Martin 161-02782-JML/GEC U. S. Nuclear Regulatory Commission January 19, 1990 Page 2 (1) APS would review the procedures supplied for the October 1989 Requalification Program Evaluation and provide replacement copies of revised procedures that would affect the simulator evaluations.

Attachment A contains an index of approved procedures requested by Reference (A), identified during the above review. The procedures are contained in two volumes and are appended to this letter.

(2) APS would supply a new set of simulator scenarios which will comprise the Scenario Exam Bank. These scenarios are provided as Attachment B; however, they are considered examination items and have been forwarded directly to Mr. Royack as confidential information in a separate mailing.

(3) APS would supply, for each person involved in examination preparation, a current "Pre-Exam Security Agreement." Attachment C contains a signed copy of a Pre-Exam Security Agreement for the upcoming simulator examinations. This agreement, signed by a member of the PVNGS Training Staff and effective 60 days prior to the examination date (or as determined by the Chief Examiner), is provided as required by NUREG-1021, Revision 5, "Operator Licensing Examiner Standards."

In addition to the above attachments, APS is supplying the following suggested crew complements for the simulator examinations. These suggested crews enable PVNGS to support the examinations and minimize impact on the Operations Shift schedules. They are considered conditional pending NRC approval.

(1) In support of the examination for Mr. M. D. Garnes, his normal shift crew will be utilized. This shift crew is scheduled for training during the week including the dates of the examination.

The crew consists of: J. H. Hunter M. A. Sanchez M. E. Barron R. W. Gulick (2) In support of the examination for Mr. M. L. Colvin, a group of NRC Licensed Nuclear Instructors will be utilized. Each of the Instructors holds a valid license, is current in requalification training, and was previously a Licensed Operator at PVNGS.
