NOTICE OF VIOLATION lilinois Power Company . Docket No; 50-461 Clinton Power Station - License No. NPF-62 During an NRC inspection conducted on May 24 through July 7,1997, four violations of NRC requirements were identified, in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG 1600 (66 FR 34381; June 30,1995), the violations are listed below:

1. Technical Specification 5.4.1.a, requires that written procedures be established, implemented, and maintained covering the activities described in Regulatory Guide 1,33,

" Quality Assurance Program Requirements," Revision 2, Appendix A, February 1978.

A. Regulatory Guide 1.33, Appendix A, Section 4. w. (2) requires, in part, procedures for energizing and operating the onsite electrical systems.

Cunton Power Station Procedure CPS 4201.01," Loss of DC Power,"

Checklist C001, " Loss of 125VDC MCC 1 A (1DC13E) Load Impact List," required that electrical diagrams be referred to as needed for detailed evaluation of the event.

Contrary to the above, on Jurie 24,1997, electrical diagrams were not referred to for detailed evaluation of a ground on safety related Motor Control Center 1 A.

The failure to review the applicable electrical diagram resulted in an inadvertent isolation of a reactor water cleanup system containment isolation valve, B. Regulatory Guide 1,33, Appendix A, Section 9.e. requires, in part, that general procedures for the control of maintenance, repair, replacement, and modification of work be prepared.

Clinton Power Station Procedure 1014.03," Temporary Modifications," Revision 18, Section 8.1.1, required that a Temporary Modification Permit be used to initiate temporary modifications.

Contrary to the above, as of June 18,1997, a Temporary Mod;fication Permit had not been initiated for a temporary modification which installed lighting that breached the main control room environmental boundary.

This is a Severity Level IV violation (Supplement 1).

2, 10 CFR Part 50, Appendix B, Criterion VI, ' Document Control," requires, in part, that measures shall be established to control the issuance of documents, such as lostructions, procedures and drawings which prescribe all activities affecting quality.

These measures shall assure that documents are reviewed for adequacy and approved for release by authorized personnel and are distributed to and used at the location where the prescribed activity is performed.

Clinton Power Station Procedure 1005.03," Control. Distribution and Statusing of Station Procedures and Documents," Revision 17, Section 8.2, required that only controlled, official working or approved for work copies of Clinton Power Station procedures be 9710290089 971014 PDR ADOCK 05000461 G PDR

Section 3.4 specified that holders of documents are responsible for maintaining operating manuals current in accordance with Records Management System Standard 4.01, " Standard for the Receipt, Control, Distribution, and Maintenance of Controlled Documents."

Records Management System Standard 4.01, Section 3.9.1.b stated that documents requested for a work activity shall be processed by Document Control as followe Documents shall be stamped " Approved For Work" in red ink on the controlling page.

- Contrary to the above, on June 20,1997, controlled official working, or approved for work copies of procedures were not used during maintenance on the Division 11 power inverter, This is a Severity Level IV violation (Supplement 1).

3. License Condition 2.E, requires, in part, that the licensee implement and maintain in effect all provisions of the Commission-approved. physical security plan including amendments made pursuant to provisions of the Miscellaneous Amendments and Search Requirements revisions to 10 CFR 73.55 and to the authority of 10 CFR 50.90 and 10 CFR 50.54(p).

Clinton Power Station Physical Security Plan, Revision 13, Section 4.3.1, requires the minimum illumination level throughout the protected area be 0.2 foot candles.

Contrary to the above, on June 19,1997, the minimum illumination level throughout the protected area was not 0.2 foot-candles in that five areas adjacent to radioactive waste storage containers were between 0.054 and 0.146 foot-candles.

This is a Severity Level IV violation (Supplement 111).

Pursuant to the provisions of 10 CFR 2.201, Illinois Power Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN:

Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region 111, and a copy to the NRC resident inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the vioiation, or, if centested, the basis for disputing the violation, (2) the corrective step that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response, if an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be give to extending the response time.

Notice of Velation -3.-

Because the response will be placed in the NRC Public Document Room (PDR), to the extent possible, it should not include any personal privacy, proprietary, or safeguards information so that -

it can be placed in the POR without redaction. However, if it is necessary to include such -

information, it should clearty indicate the specific information that should not be placed in the PDR, and provide the legal basis to support the request for withholding the information from the

- public.
