NOTICE OF VIOLATION Illinois Power Company Docket No. 50-461 License No. NPF-62 As a result of the inspection conducted on December 17, 1990, through January 30, 1991, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, (1990)
(Enforcement Policy) the following violation was identified:
10 CFR Part 50, Appendix B, Criterion Ill, Design Control,.'equires that measures be established to assure that the design basis is correctly translated into specifications, drawings, procedures and instructions.
It further requires that the design control measures provide for verifying or checking the adequacy of the design, such as, by performance af design reviews, by the use of alternate or simplified calculational methods, or by the performance of a suitabl.e testing program.
Contrary to the above requirements, the licensee failed to ensure that adequate design control measures were established to support field alterations, as evidenced by the following examples:

. Calculations performed on October 1,1985, January 20, 1986, and 11 arch 27,1990 were not verified to ensure the adequacy of the design.
Further, discrepancies between the referenced calculations and previous information, in the Final and Updated Safety Analysis Report, were not evaluated to ensure that the design basis was correctly translated into spe:ificatiens or instructions.
B.
For Field Alteration I A-F010, which modified llSIV, SRV, and ADS valve accumulator piping configurations, the licensee failed to ensure that the design and post-modification testing criteria were adequate. The design verification was deficient, in that:
(1) incorrect piping matericl weights were used in the seismic calculation, and the documented; (2) justification for not performing system flushing, as required by CPS 2800.04, was not documented; and (3) the post-modification functional (air leakage) testing criteria was without technical basis.
This is a Severity Level IV vio4 tion (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation:
(1) the corrective steps that have been taken and the results achieved; (2) the
' corrective steps that will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
