i NOTICE OF VIOLATION Illinois Power Company Docket No. 50-461 Clinton Power Station License No. NPF-62 As a result of the inspection conducted from July 6 through August 24, 1993, and in accordance with 10 CFR Part 2, Appendix C - General Statement of Policy and Procedure for NRC Enforcement Actions, the following violations were identified:
Criterion XVI of 10 CFR 50, Appendix B, requires that measures be established to assure that conditions adverse to quality, such as failures and deficiencies, are promptly identified and corrected.
In the case of i
significant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition.
Contrary to the above:
A.
The licensee identified and documented more than a dozen pipe support deficiencies in the RHR systems "A" and "B", including damage to four rigid pipe restraints, in condition reports (CRs) dated January 8 and l
January 17, 1991. The CRs were closed without adequate assessment to determine the root cause(s) and safety significance (461/93010-03A(DRS)).
B.
Cracked welds were identified and documented on the RHR pump "B" discharge pipe guide in a CR dated February 5, 1991. The condition report was closed without inspection of similar pipe guides and assessment of the safety significance (461/93010-03B(DRS))
These violations represent a Severity Level IV problem (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Illinois Power Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region III, and a copy to the NRC i
Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of i
Violation" and should include for each violation:
(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
If an adequate reply is not received within the time specified in this Notice, an order or a Demand for information may be issued to show cause why the license should not be j
modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
