Subject: NOTICE OF VIOLATION (Inspection No. 50-334/87-10)

This refers to the NRC resident safety ir- .ction conducted on June 5-17, 1982 at the Beaver Valley Nuclear Power Stati , Unit Shippingport, Pennsylvani The inspection report was sent to you on vune 23, ;987. The inspection was conducted to review the circumstances associated with a violation of a tech-nical specification limiting condition for operation (LCO) which was identified by your staff and reported to the NR During the inspection, an additional violation of NRC Quality Assurance requirements was identified which contrib-uted to the violation of the LCO. On July 2, 1987, an enforcement conference was conducted with you and members of your staff ' discuss the violation, its causes, and your corrective action The violations are deicribed in the enclosed Notice. The first violation involved the inoperability of all three chlorine detection system The systems had been inoperable since initial installation in 1980 in that they would not have obtained a representative sample of control room intake air because of inleakage into the sampling line from adjacent spaces. In this condition, the chlorine detection systems would not perform their intended safety function of automatically isolating the ventilation system and placing the control room air circulation in a recirculation mode, in the event of tigh chlorine levels in the control room ventilation system intake ai The NRC recognizes that the safety significance of this condition was mini-mized by the fact that (1) the chlorine detectors would have sensed levels in the ventilation equipment room (which is part of the control room ventilation envelope) and been activated when overall chlorine concentration levels exceeded their trip setpoints and apparently prior to toxic concentrations reaching the control room operators; (2) procedures require, in the event of a high chlorine level, that a control room operator manually isolate the ventilation system and transfer circulation to the recirculation mode; and (3) the distance between the only plausible source of chlorine in the chlorination building and the control room air intake makes any significant chlorine intake unlikel i Nonetheless, the NRC is concerned that the design of these chlorine detection '

systems was inadequate, and post-installation testing of the systems was not sufficient to detect this design error. As a result, the systems did not meet the configuration described in the FSAR in that the chlorine detectors would not have received a representative sample of the control room air intak These events demonstrate the importance of proper design and suitable testing when modifications are made at the facilit Therefore, although the violations described in the enclosed Notice have been classified in the aggregate at Severity Level IV in accordance with the
