Remo electrical penetration connections to field'wirino, inside and outsida containment. The deficiencies in the cable splices were discoverad by Daqion- ~

r IV as a result of'their inspection activities at Comanche Peak Station. The following sumnarizes our findings and proposed resolution of the problem. .

Field wiring connections to the electrical penetrations conductors are soliced ,

twice for all power, control and instrumentation circuits inside as well as out-

side contairments The vendor-installed splices are with'in 17 inches of the electrical penetratine assemblies (EPA) and the field splices are installed -

in the raceways at a distance of_ approximately 6' from the EUA. Cable solices '

~

in the raceways are prohibited by position C9 of R.G.1.75 because they have '

been identified as the' initiating cause of several fires in raceways. The- -

Comanche Peak design is inconsistent with the recormendations of -this guid*,

It is-not clear at this point why the double splicing became necessary.

Possihiv the field cables were not long enough to terminate at the EPA or tha vendor did not supply long penetration conductor pigtails for field connections.

In any event this situation is very unique and certainly not ocod encineerino practice. .

!^ ' .were inadeoustely installed in that both ends of the splices were not proper- 3

yQ ly sealed, b) the cables with solices were not proper 1v routed, c) the splices h were not staogered within the racewavs (splices were stacked on too of each -

.other), and d) the factory instailed cable 501(ces were also not proper 1v sealed

and.in'some cases bare conductors were visible.

.The deficiencies in the installation of cable splices could result in failure '

plant. . The potential failures which could occur include electrical'short cirp 2

brated tools should provide a solice tha* could be as good as the original F cable. Therefore, in order for us to eccept the use of cable splices in the '

raceways, the applicant must develop a program to inspect all solices (field _ Av

. i-as wall as factory installed) located inside and outside containment for 1 connecting field wiring to the. electrical penetration conductors.

ment of such solices. Howev$r. such replacement must be. supported by documenta M

i tion containing the results of qualification tests conducted to support these I corrective actions. This should include appropriate environmental c'ualifica-1- , -

tion,as well as electrical continuity tests, s r -

virr,nmental qualification. inspection and installation'and electrical continu ~ g' ity tests results. For added assurance we reoutre -that the cable splices of' all medium voltage and low voltage must be capable of carrving rated continuous : . -

current prior to and following rated short circuit current without the terperature of the splice exceeding the temperature of the conductor.
