TRIP REPORT - DESIGN ACTIVITY HOMOGENEITY i DOCUMENTATION AUDIT, COMANCHE PEAK PROJECT On December 2 and 3,1985 representatives of the NRC visited the TERA offices in Bethesda, Maryland to audit the documentation TERA had on file to support an initial level of homogeneity for Design Activities. The scope of the Design Adequacy Program (DAP) plan stated that each Design Activity exhibits five elements of homogeneity. These elements are:

5) Consistent design interfaces throughout the design effort.

Additionally, the audit was to review the occupants of Design Activities and discuss the process for selecting occupants to be inspected and review the procedure (DAP-21) that will be implemented to validate homogeneity for each Design Activity.

During the audit the following discussions took place and documents were reviewed:

1. An introductory discussion pertaining to the documentation that will be reviewed was held.
2. The G4H calculation and specification lists cross-referenced to the applicable Design Activities was reviewed as the documentation basis for occupants of design activities.
3. A discussion was held within the Mechanical discipline group pertaining to the homogeneity element of methodology. No documentation in the form of technical design instructions or standards were offered for review. The discussion revolved around the rationale that the Design Activities were of such a narrow scope that a variation in methodology could not exist.

It was further discussed that the design criteria contained methodology; but, no documentation was presented in support of this contention.

l' 4. TERA presented correspondence from G8H that showed a consistent project organization over the design period. These documents were reviewed. This infomation was also presented as the basis for consistent project design t interfaces.

, 5. In the area of design process control, corporate G8H procedures for design control (DC-1 thru -13) and project control (PC-1 thru -9) were reviewed.

These procedures set forth prescriptive instructions for the project to develop certain technical design documents (i.e. Design Descriptions and Interfaces). These procedures also control the approach to perfoming calculations. At the time of this audit TERA did not present any of the items that were developed by these procedures for the Comanche Peak Project.

6. A discussion was held concerning the utilization of the DAP procedure (DAP-21) for validation of homogeneity for each Design Activity. Assuming

'an initial level of homogeneity'the procedure requires the selection of five occupants of each Design Activity, equally spaced in time (over the design period). These samples will be scanned for consistency of crite-ria, methodology and application of design control procedures. The other two elements of homogeneity (organization, interfaces) will be validated on a progranaatic basis. If homogeneity cannot be validated the Design Activity will be broken down into smaller Design Activities that possess homogeneity or there will be 1005 inspection of the occupants.

7. A discussion was held concerning the process for selecting occupants for inspection. The discussion entailed a concern for triviality, e.g. if the occupant within the AFW system presents a trivial case, another more complex occupant would be selected from another system.
8. TERA declared their intent to exclude prequalified design agents and suppliers, such as Westinghouse from the HDA review process. The basis for this position was that these agents have demonstrated satisfactory paper controls and QA/QC practices in other projects. This should com-

. prise satisfactory product. These selections would be documented in an Engineering Evaluation.

9. A brief discussion was held concerning the situation where Westinghouse has design responsibility for a safety system. The responsibility is typical to what an A/E has for approving the adequacy of construction documents by Proof-of-Design calculations. It was indicated that on Comanche Peak. Westinghouse has this type of responsibility for the Chemical and Volume Control system (CVCS) and the Residual Heat Removal system (RHR). 10. In the electrical engineering and control and instrumentation disciplines a number of design information documents were sighted exhibiting methodo1-ogy. These covered subjects on cable sizing, generator short circuit current contribution, circuit breaker selection, instrumentation and control diagram development, and generator loss of field protection.

Also, several electrical engineering calculations were performed by use of computer programs. The related computer program user guides or instruc-tions were not sighted.

11. In the civil and structural discipline a number of 6&H design documents were presented which appeared to exhibit methodology.
