Haddam Neck Plant response to Open items Identified During the May 8,1986 Meeting on a Proposed Revision to the Inservice Testing Program (IST)

This letter is being forwarded in response to Reference (1) which documented the results of a working meeting held on May 8,1986 between the Connecticut Yankee Atomic Power Company (CYAPCO) and NRC representatives to discuss questions resulting from the review of a proposed revision to the Haddam Neck pump and valve inservice testing program.(1)(2) The meeting minutes, as documented in Reference (1), identified 11 remaining open items. Subsequent telephone calls, held on May 13 and 14,1986, resolved 5 of the open items. This is documented in Enclosure 2 to Reference (1). Additionally, questions not formally documented, but handed out during the course of the May 8,1986 meeting were discussed during these telephone conversations. Again, Enclosure 2 to Reference (1) documented these results and showed all but one of these concerns were resolved.

Attachment 1 is being provided in response to the remaining open items identified in Enclosures 1 and 2 of Reference (1). Items agreed upon as closed have been restated while all remaining open items are addressed by reidentifying the original open item and providing a response.

Provide a more technical justification for not full stroke exercising valve SI-CV-103,107A & B, and SI-CV-872A & B in accordance with the code specified frequency (with the maximum time interval being two years).

Response

Connecticut Yankee will full stroke exercise these valves each refueling at less than full flow and once every five years at full flow. (Item remained open as documented in Enclosure 2 to Reference (2).)

The Licensee would like to perform a full flow test once every 5 years.

Development of pump curves would be used to demonstrate operability of valves in question at less than full flow. It was agreed that a written description and technical evaluation will be provided to the NRC for their review.

Response

Technically this was found acceptable.

Each pump is designed to pump flow against a specific resistance which is called the pump design point. Any change in system resistance (pipe friction or valve position) would cause a change in pump head and flow, as defined by the pump curve.

FLOG fi e e. 1 Point 1 in the above Fig. I represents a valve position near its closed position. If a check valve failed to open, the increased system resistance would produce a high pump head. Point 2 in the above Fig.1, represents a valve position near its fully open position. If a check valve functioned properly, there would be very little increase in system resistance and therefore a lower pump head would be produced. By increasing the pump head, we can determine pump flow and verify valve operability.

Every five years we will develop the pump curve shown in Figure 1, and evaluate it per its design point.

Each system would be lined up to inject water into the Reactor Vessel.

One pump will be started from each system, taking water from the RWST and passing through its correspondent check valve and piping into the vessel. The discharge valve of each pump will be open until the pump head, (which will be calculated based on the discharge and suction pressure),

corresponds to a desired flow based on the pump curve developed during the five year system test. The reactor vessel head is vented to the pressure relief tank by removing one of the code safeties and adding a spool piece connecting the piping. The pressurizer will be filled and drained several times during this test. The level in the pressurizer will not exceed 90% level. A copy of the system flow test and procedure is included as Attachment 3 to this submittal.

The staff normally accepts no relief request from frequencies greater than 2 years. The Licensee should investigate alternate means of -

complying with the Code specified frequency including design changes, alternate flow paths to maintain decay heat removal, including necessary Technical Specification changes.

Response

The Licensee concluded that design changes or Technical Specification changes to mect the IST program are not warranted.

Alternate means of testing were reviewed. However, no alternate means were found acceptable.

The Licensee concluded that its recommendation per Reference 2, is an acceptable solution to the test frequency.

Both valves have been tested, RH-CV-783, on February 27,1979,and RH-CV-808A on February 16, 1983. The results of these tests found both valves satisfactory and no internal degradation was found. A copy of the inspection procedure is attached for your review as Attachment 4 to this submittal. Both valves are scheduled for inspection during the 1987 outage.

During the NRC Review there was an additional listing of valves provided to the Licensee for explanations as to why they are not in the IST program.

Valves that need final explanations are listed below. Any relief requests

required are identified within the IST Program under the applicable section.

1) RC-MOV-501 RC-MOV-526 RC-MOV-512 RC-MOV-537 RC-MOV-513 RC-MOV-533 RC-MOV-524 RC-MOV-546 These valves will be added to the IST Program as Class 1, Category B Valves, and will be tested during each refueling.

This is a change in our previous position, as stated in Reference (2). Test requirements would be to exercise the valve and verify Position Indication. These valves will be closed 30 minutes after steam generator tube rupture by operator action.

2) RC-CV-509 RC-CV-423 RC-CV-536 RC-CV-545 These valves will be added to the IST Program as Class 1, Category B Valves. These valves provide loop over pressure protection. Due to geometry there may be no practical way to exercise these valves.
3) CC-RV-763A, B These valves will be added to the IST program as Category C Valves. This is a change from our previous position, as documented in Reference 2.

OPERATIONAL READINESS TESTING FOR VALVES The attached t'a ble includes all valves in the Haddam Neck Plant which are required to be identified under Subsection IWV of ASME Section XI, as further defined in NRC staff guidelines and in meetings with NRC etaff reviewers, CYAPCO, and NUSCO personnel.

Most of the valves are presently under surveillance testing requirements of plant Technical Specifications or administrative 1y controlled by plant procedures. In some cases, the specific requirements of IWV cannot be met due to plant design, operational requirements, or infringement on safety margins. These situations are noted as required and pertinent waiver requests and alternate test schemes are presented.

Valves which cannot be tested while the reactor is at power can, for the most part, be tested during cold shutdowns. CYAPC0 has established a program to commence testing these valves within 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> of achieving cold shutdown conditions and to continue as time permits on subsequent shutdowns without repeating valve tests until the entire list has been completed.

Leak testing of pressure isolation valves will be conducted to the requirements of IWV-3420.

Relief valve setpoints will be tested in accordance with ASME PTC25.3 -

1976.

In that the Haddam Neck Plant has been in service for over ten years before the start of this program, the preoperational test data required by IWV-6220 are not available in all cases. Component record packages will contain available preoperational and manufacturer's test reports.

This information is located, in most cases, in Nuclear Records.

All exercising and testing procedures required by Article IWV, including schedules and the limiting values of observed parameters, shall be defined and performed by CYAPCO.

Locked valves are listed separately. These valves are considered

" passive" and, as such, are not exercised or tested.

Operation of these valves could cause flow to be lost in the loop and would isolate a steam generator, causing a plant shutdown and loss of a heat sink.

Alternate Testing: These valves will be tested during refueling.

SYSTEM: SAFETY INJECTION

1. Valves: SI-CV-103, 107A, 107B, and SI-CV-872A/B Class / Category: 2-C, 1-AC Function: Check valves in flow path of LPSI pumps to core deluge valves.

Test Requirement: Exercise valves to full open position.

Basis for Test Frequency: Full or partial valve exercising requires that flow be established into the reactor vessel from the refueling water storage tank, using the LPSI pumps. System pressure downstream of these valves does not allow flow during normal operations.

During cold shutdown, available volume in the reactor vessel is insufficient to accommodate flow required for exercising these valves. Introduction of RWST water to the reactor coolant system requirca additional waste liquid processing which is time consuming and delays start-up.

Alternate Testing: CYAPCO will perform a system flow tert each refueling to exercise these valves. The water will be pumped from the RWST to the reactor vessel. The flow will be determined by measuring the discharge and suction head. CY will calculato the pump head to find the corresponding flow from the pump curve that indicates valve perf ormance.

Basis for Test Frequency: In order to full stroke these check -

valves, flow must be established into the reactor vessel from the refueling water storage tank using the HPSI pumps. System pressure downstream of these valves does not allow flow during normal operation.

During cold shutdown, available volume in the reactor vessel is insufficient to accommodate flow required for exercising these valves. Introduction of RWST water to the reactor coolant system requires additional waste liquid processing which is time consuming and delays start-up.

Alternate Testing: CYAPCO will perform a system flow test each refueling to exercise these valves. The water will be puuped from the RWST to the reactor vessel. The flow will be determined by measuring the discharge and suction head. CY will calculate the pump head to find the corresponding flow from the pump curve that indicates valve performance.

3. Valves: SI-MOV-861A/B/C/D Class / Category: 1-A Function: Pressure isolation stop valves in flow path f rom IIPSI pumps to reactor coolant system.

Test Requirement: Exercise to full open position.

Basis for Test Frequency: The inability to check the closed integrity of SI-CV-862A/B/C/D prior to exercising SI-MOV-861A/B/C/D.

These valves interface directly with the RCS at operating pressure.

Alternate Testing: These valves will be full stroked at cold shutdowns.

1. Valves: RH-MOV-23 and RH-MOV-34 Class / Category: 2-B Function: Isolates containment spray system from RHR system.

Test Requirement: Exercise valves for operability.

Basis for Test Frequency: The containment spray system is set up such that opening either RH-MOV-23 or 34 will initiate flow. By entering the containment and closing manual valve RHR-V-23A, the valves could be "no flow" stroked, but technien1 specifications 3.11 prevent this, as it would be dis-abling the containment spray system.

These valves will be exercised during cold shutdowns.

2. Valve: RH-MOV-33A/B Class / Category: 2-B Function
  • Isolates cross-connect between RHR system and charging system.

Test Requirement: Exercise valve for operability.

Basis for Test Frequency: RH-MOV-33A/B provide a flow path from the RHR system to the charging pump suction header. Stroking these valves varies the suction header pressure in the charging system and could cause charging pump flow oscillations.

These valves will be exercised at cold shutdowns.

3. Valves: RH-MOV-780 and 781 Class / Category: 1-A Function: Exercise valve for operability.

Basis for Test Frequency: Stroking these valves requires that the service water system be cross-connected with the component cooling water system to provide emergency flov to the residual heat removal heat exchangers. The service water system utilizes water from the Connecticut River. The component cooling water system is a closed system containing potentially contaminated water. Cross-connect-ing could result in releasing nonconforming water to the river and require a major cleanup of system components, which would require that they be taken out of service during the restoration process.

Alternate Testing: We propose to exercise these valves each refueling.

l SYSTEM: VARIOUS CONTAINMENT ISOLATION

1. Class / Category: 2-A, 2A-C Function: To insure containment integrity in the event of a requirement to isolate.

Test Requirement: Exercise all active valves and determine leak tightness.

Basis for Test Frequency: This list consists of valves whose' only safety feature is containment isolation. Those containment isolation valves which are used also for pressure isolation are listed under the particular system in which they operate. Pressure isolation valves are tested in accordance with IWV-3420. Operability testing of these valves during normal plant ,

operation could cause a loss of containment integrity and/or system function if a valve failed in a nonconservative position. Present technical specifications and associ-ated procedures require both an individual and an integrated leakage rate test for containment isolation valves and penetrations. Procedures also require repair of any valve exceeding the leakage limit. The sum of leakage from all penetrations and isolation valves is used to determine an integrated leakage rate which is compared to the maximum allowable leakage rate stated in the technical specifications.

Alternate Testing: CYAPCO proposes to use existing procedures for leak testing of valves used as containment isola-tion. Modifications to procedures, as agreed upon by CYAPCO and NRC staff, concerning " Appendix J" tests

. and exemptions, will be reflected in this program. All " active" contain-ment isolation valves are demon-strated operable during cold shut-down or reactor refueling at least ohce each 18 months, by verifying that all valves actuate on contain-ment isolation signals in accordance with plant technical specifications.

Passive valves do not require exercising.

SYSTEM: CHARGING RCS

2. Valves: CH-CV-399 Class / Category: -

1-AC Function: Changing to Reactor Coolant System and insure containment integrity in the event of a requirement to isolate.

Test Requirement: Test valve to determine leak tightness.

Basis for Relief Request: CYAPC0 had previously requested an exemption from the Type C testing requirements of Appendix J for the reactor coolant charging (P-8) penetration. This previous report was based on the seismic design of system piping inside containment and the proposed seismic qualification of system piping from the isolation valves of Penetration P-8 to its water source. Subseauent evaluations determined such qualification to be a lengthy and costly effort; therefore, CYAPCO intends to modify Penetration P-8 to permit Type C testing. In the interim, although Penetration P-8 is not currently Type C tested, the isolation valves in this penetration are exposed to water subjected to containment pressure through the vented reactor coolant system during the CILRT.

The valves are exposed to containment pressure in the direction of accident pressure with the back side of the valves depressurized. Further, the portion of the system outside containment is checked for 11auid leakage in accordance with Administrative Technical Specification 3.14; system leakage is accounted for in off-site dose consequence calculations, per 10CFR100. Such leak testing provides additional assurance that the potential for significant containment atmosphere leakage through this penetration is minimized.

CYAPCO believes a schedular exemption from the Type C testing requirements of Appendix J is justified based on the above information, as well as the long lead time for equipnent needed to modify this penetration, the level of design and analysis required, and the need to integrate these modifications with the ultimate resolution of the SEP review of containment isolation valve configurations per 10CFR5 .

Appendiy A. This approaci is consistent with the ALARA philosophy in that it avoids repetitive modifications to the same penetration. The ultimate resolution of this issue will be accomplished within the framework of the Haddam Neck Plant ISAP.

3. Valves:- SI-V-860 Class / Category: 2-A Function: Safety injection to reactor cavity fill.

Test Requirement: Test valve to determine leak tightness.

Basis for Relief Request: Valve SI-V-860 is a 3" locked close manual gate valve that isolates the cavity fill line. The cavity fill line branches off the high pressure safety injection header inside containment. This valve is not LLRT tested, but is tested during the ILRT in the accident direction with air on the containment side of the valve. The downstream side of the valve is covered with liquid, but i

vented during the ILRT. This is consistent with expected post-LOCA conditions because valve SI-V-860 is in a vertical run with liquid trapped above it due to testing of the high pressure safety injection system.

The recent PRA analysis of the penetration resulted in a total risk (due to noncompliance with Appendix J) of 0.9 man-rem over the life of the plant. Of the total risk, 0.3 man-rem is due to valve SI-V-860 not being tested. It is assumed that SI-V-860 leaks I

volume per day, which is greater than twice the worst measured containment isolation valve as-found leakage at the plant. Clearly, the

! risk due to this untested valve is very small. Since the ILRT does leak test this valve, the actual risk will be much less than

REVIEW LEAK OF TESTING REQUIREMENTS FOR PRESSURE ISOLATION VALVES ST-CV-862A/B/C/D - Valves are subjected to type "C" test as containment isolation check valves and a 150# 1eakage test for pressure isolation valves.

SI-V-863A/B/C/D - Valves are subjected to a 150# 1eakage test for pressure isolation valves and a Type "C" containment isolation check valve.

SI-MOV-861A/B/C/D - Each valve is subjected to a 150# 1eak test.

SI-MOV-871A/B and SI-CV-872A/B are velded to one another with no pro-vision for leak testing CD-MOV-871A/B using reactor coolant pressure.

During the system hydrotest at startup, SI-MOV-871A/B are open, thus, subjecting SI-CV872A/B to reactor coolant pressure between 350-375 psig.

Leakage is measured at that time.

l RH-MOV-803,804,780,781_ are individually leak tested during the system hydrotest using reactor coolant at 350-375 psig.

l FH-MOV-578-535-522-508 piping is designed for reactor coolant tempera-l ture and pressure heck to charging pumps. No tests are made.

FH-CV-296 is subjected to a Type "C" test for containment isolation valves. Piping is designed for reactor coolant temperature and pressure, back to the charging pumps. No pressure isolation test is required.

DH-MOV-562,507,521,534,544 and DE-V-502,516,529,539, and 525 are not individually tested. If leakage occurs, it is cooled by the drain cooler and piped back by way of FH-MOV-310 or FH-V-311 to either the volume control tank or to the primary drain tank, both of which are monitored and alarmed at the main control board. These valves are not required to be tested.

FH-MOV-310,FH-V-311 are individually leak tested at 150# for pressure isolation valves.

Loop fill and loop drain valves (FH and DH) are listed in Table IWV.

These valves are not used in bringing the reactor to a safe shutdown condition and are not normally used to mitigate the consequences of an l

l accident, however, these valves (fill only) may be used as backup for small break LOCA. These valves are normally used to fill and/or drain loops during refueling operations.
