Proaram Report on an Unsatisfactory Performance Testino incident l Pursuant to 10 CFR 26, Appendix A, Subpart B, Section 2.8 (e) (4), Northeast Utilities hereby submits a Fitness For Duty (FFD) Program report for investigations involving unsatisfactory performance test results (" false negative"). These test results are from a Department of Health and Human Services (DHHS) certified laboratory under contract #

to Northeast Utilities to perform drug testing as required by 10 CFR Part 26 in support of Northeast Utilities FFD Program. j On July 17, 1998, Quest Diagnostic' Incorporated processed a blind specimen and initially reported it as negative and subsequently determined it to be positive upon re-testing. Attachment 1 provides Quest Diagnostic's report of its investigation of this t incident, dated July 24,1998. As stated in their report, individuals who were involved in this incident have been re-trained. i As tabulated in Attachment 2, there are no commitments contained in this letter.

Dear Dr. Millette:

In response to the request received from Ms. Cora Quinn-Ross on 7/22/98, the laboratory conducted a full investigation of specimen T84910. After extensive and complete review of the analytical records, the laboratory has determined the following.

Specimen T84910 was received by Quest Diagnostics Inc. on 7/17/98. The specimen was screened positive for amphetamines by the Enzyme Multiplied Immunoassay Technique (EMIT) test on 7/17/98.

The confirmation test for amphetamines by Gas Chromatography / Mass Spectrometry (GC/MS ) on 7/17/98, produced what appeared to be a negative result for both amphetamine and methamphetamine.

Accordingly, the specimen was certiDed and reported to you as negative.

Further examination of the GC/MS chromatographic data revealed that amphetamine was indeed present in that specimen, howe"er, the presence of the MDMA in the specimen caused a shift in the retention time thi.t prevented the positive identification of amphetamine.

As part of the corrective action, the tape of the GC/MS analy tical batch that included this specimen was re-loaded on the GC/MS instrument. The chromatographic data for specimen T84910 was re processed and the time window was adjusted to accommodate for the shift in the retention time caused by the MDM A.

As a result, a positive identification for amphetamine at a concentration of 719 ng/mL was obtained. A corrected certified laboratory report has been issued and a copy is attached with this letter.

Also, all of the individuals who were involved with the review and certiDcation of the results for this specimen have been re-trained in the proper procedure for identifying amphetamine / methamphetamine in unusual specimens.
