No. 12 Steam Generator Pressure Remote Shutdown Indication -

Inoperable.

This report was initiated by Incident Report 82-221.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0 % - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

At 2330 hours0.027 days <br />0.647 hours <br />0.00385 weeks <br />8.86565e-4 months <br />, August 9, 1982, during routine operation, a technician discovered that the No. 12 Steam Generator Pressure indication was reading zero at the remote shutdown panel; actual pressure was 1005 psig. The channel was declared in-operable, and Technical Specification Action Statement 3.3.3.5.a was entered.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The loss of No. 12 Steam Generator Pressure remote shutdown indication was due to the failure of a transistor in the instrument power supply. No recent failures of the power supply have been noted, and the problem was assumed to be of an isolated nature.

ANALYSIS OF OCCURRENCE:

Remote shutdown instrumentation is a design feature that ensures the facility can be shutdown and maintained in hot standby in the event of a loss of control room habitability.

The feature is in addition to Control Room design features, including shielding, non-combustible materials, fire barriers and air conditioning systems. These provisions ensure occupancy of the room under all conditions, including accidents analyzed l in the FSAR. No degradation of Control Room design features or habitability was involved in the occurrence. The incidence, therefore, did not involve any risk to the health and safety of the public. The event constituted operation in a degraded mode permitted by a limiting condition for operation, and is reportable in accordance with Technical Specification 6.9.1.9.b.

Action Statement 3.3.3.5.a requires:

With the number of operable remote shutdown monitoring channels less than required by the Technical Specifications, either restore the inoperable channel to operable status within 30 days or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CORRECTIVE ACTION:

The failed transistor was replaced and the channel calibration was satisfactorily performed. No. 12 Steam Generator Pressure remote shutdown channel was declared operable at 1625 hours0.0188 days <br />0.451 hours <br />0.00269 weeks <br />6.183125e-4 months <br />, August 10, 1982, and Action Statement 3.3.3.5.a was termi-nated. No further action was deemed necessary in view of the nature of the failure.
