2C Vital Bus Undervoltage Relay - Inoperable.

This report was initiated by Incident Report 82-017.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 94% - Unit Load 1090 MWe DESCRIPTION OF OCCURRENCE:

On January 21, 1982, during the performance of the M3T maintenance surveillance test, it was discovered that the undervoltage relay on the 2C Vital Bus was actuating at 55.5 volts instead of the 77.3 volt trip setpoint value.

The relay was placed in the tripped condition, and-at 2050 hours0.0237 days <br />0.569 hours <br />0.00339 weeks <br />7.80025e-4 months <br /> Action Statement 3.3.2.a Action 14 was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.a.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The undervoltage relay actuated at 55.5 volts instead of the 77.3 volt trip setpoint value.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.2.a Action 14 requires:

With the number of operable channels one less than the total number of channels, operation may proceed until performance of the next required channel functional test, provided the inoperable channel is placed in the tripped condition within one hour.

CORRECTIVE ACTION

Due to a similar occurrence with the same relay two months earlier, it was inspected by both the Relay Department and the Energy Lab.

No defects were found.

The relay was recalibrated and tested satisfactorily.

The M3T surveillance test was satisfactorily performed.

At 1040 hours0.012 days <br />0.289 hours <br />0.00172 weeks <br />3.9572e-4 months <br />, January 23, 1982 Action Statement 3.3.2.a was terminated.
