Fire Barrier Penetrations - Lack of Fire Watches.

This report was initiated by Incident Report 82-033.

1 CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1150 MWe DESCRIPTION OF OCCURRENCE:

At 1050 hours0.0122 days <br />0.292 hours <br />0.00174 weeks <br />3.99525e-4 months <br />, February 15, 1982, the Senior Shift Supervisor was informed by'the NRC Resident Inspector that there were two 4 inch core bores opened on the 78' Elevation between the r

Mechanical and Electrical Penetration Areas, and no fire watch i

was posted.

That constituted a violation of Action Statement 3.7.11, which was entered in both units, already utilizing numerous fire watches.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel error..The fire watch was inadvertently posted on the No. 1 Unit 78' Elevation Mechanical-Electrical Penetration i

Area instead of No. 2 Unit.

Technical Specification 3.7.11 requires:

With one or more of the required fire barrier penetrations

inoperable, within one hour either establish a continuous fire watch.on at least one side of the affected penetration, or verify the operability of fire detectors on at least one I

CORRECTIVE ACTION

A fire watch was posted immediately.

The supervisor in charge was advised of the fire watch omission and stated that the watch went to the wrong unit.

Subsequently, the fire watch was correctly posted.

The NRC Resident Inspector was notified of the correction.
