No. 21 Steam Generator Level Channel 2 - Inoperable.

This report was initiated by Incident Report 82-039.

CONDITIONS PRIOR TO OCCJRRENCE:

Mode 3 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

On February 19, 1982 No. 21 Steam Generator Level Channel 2 was reading 20% higher than Channels 3 and 4.

At 1740 hours0.0201 days <br />0.483 hours <br />0.00288 weeks <br />6.6207e-4 months <br /> No. 21 Steam Generator Level Channel 2 was declared inoperable and Action Statements 3.3.1 Action 7 and 3.3.2.b Action 14 were entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION ;g' APPARENT CAUSE OF OCCURRENCE :

The sensing lines to the transmitter were plugged with suspended solids.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.1 Action 7 requires:

With the number of operable channels one less than the total j

number of channels, startup and/or power operation may proceed until performance of the next required Channel Functional Test provided the inoperable channel is placed in the tripped condition within one hour.

CORRECTIVE ACTION

The channel bistable was placed in the tripped condition within one hour.

The transmitter was tested and found satisfactory and in calibration.

The sensing lines were blown down to remove accumulated suspended solids.

Channel 2 was tested satisfactorily and returned to service At 0001 hours1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, February 20, 1982, Action Statements 3.3.1 Action 7 and 3.3.2.b Action 14 were terminated.
