IDENTIFICATION OF OCCURRENCE:

1PRl and lPR2 - Loss of Reactor Coolant System Vent Path.

CONDITIONS PRIOR TO OCCURRENCE:

J Mode 5 - Rx Power 0% - Unit Load 0 MWe l

DESCRIPTION OF OCCURRENCE:

j On March 1, 1982, the Control Room Operator released Pressurizer Relief Valve 1PR2 for service following maintenance; the valve was left in the closed position.

Valve 1PRl was then closed and tagged out for maintenance.

Operation proceeded with both valves closed until 1203 hours0.0139 days <br />0.334 hours <br />0.00199 weeks <br />4.577415e-4 months <br />, when the operator noticed that no Reactor Coolant System (RCS) vent path existed.

Action Statement 3.4.9.3.b was entered retroactive to 0856 hours0.00991 days <br />0.238 hours <br />0.00142 weeks <br />3.25708e-4 months <br />, when valve 1PRl was closed.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

4 The cause of the loss of RCS vent path was that the Control Room Operator inadvertently closed Valve 1PRl with IPR 2 still in the closed position.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.4.9.3.b requires:

CORRECTIVE ACTION

At 1203 hours0.0139 days <br />0.334 hours <br />0.00199 weeks <br />4.577415e-4 months <br /> the operator opened IPR 1, and Action Statement 3.4.9.3.b was terminated.

The operator involved was counseled by the Senior Shift Supervisor concerning Technical Specification 3.4.9.3 and the importance of monitoring plant status with respect to Limiting conditions for Operation.
