Containment Fan Coil Unit - Low Flow.

This report was initiated by Incident Report 82~021.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 96% - Unit Load 1080 MWe DESCRIPTION OF OCCURRENCE:

On January 28, 1982, upon starting No. 22 Containment Fan Coil Unit (CFCU), the service water flow indicator indicated a no flow con-dition in both high and low speea.

At 0625 hours0.00723 days <br />0.174 hours <br />0.00103 weeks <br />2.378125e-4 months <br /> No. 22 CFCU was declared inoperable and Action Statement 3.6.2.3.a was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Zero service water flow was due to service water discharge valve 22SW223 not opening in response to the CFCU start signal.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3.a requires:

With one group of cooling fans inoperable and both Containment Spray Systems operable, restore the inoperable group to operable status within 7 days or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

Service water discharge valve 22SW223 was stuck shut.

It was opened locally and exercised several times.

Normal flow indication was restored.

Surveillance Procedure 4.6.2.3 was satisfactorily performed at 1005 hours0.0116 days <br />0.279 hours <br />0.00166 weeks <br />3.824025e-4 months <br />, and Action Statement 3.6.2.3.a was terminated.
