Theoperators stabilized theplantinHotShutdownutilizing theEmergency Operating Procedures.

TheHC"MFRVwasfoundtohaveshearedanti-rotation pinsandthestemdetachedfromthevalveplug.Thecauseforthevalvefailurewasattributed tohydraulic forcesandvibration actingovertime.TheMFRVswerealldisassembled, inspected andrepairedasnecessary.

ThiseventwasreportedtotheNuclearRegulatory Commission at0300hours,inaccordance with10CFR50.72.b.2.ii.

PriortotheOperating crewassumingtheshiftat2300hourson2/11/89,the<<B<<MainFeedwater Regulating Valve(MFRV)hadpreviously exhibited erraticvalvecontrolforsteamgenerator waterlevel.The<<B<<MFRVwasintheautomatic mode,however,thevalvewasnotcontrolling smoothly.

At0048hours,operators noticederraticoperation ofthe<<C<<MFRV.At0108hours,atapproximately 554power,the<<A<<MainFeedwater Pumpwasshutdown.

At0120hours,thewaterlevelinthe<<C<<steamgenerator increased tothehigh-high levelsetpoint(75%)causing.aturbinetripandfeedwater isolation signal.Theturbinetripcausedareactortrip,sincereactorpowerwasabovethetrippermissive of49'.Thefeedwater isolation signalcausedthefollowing actionstooccur:1)tripoftherunningmainfeedwater pump,2)auto-start oftheauxiliary feedwater pumps,and3)closureofthemainfeedwater regulating valvesandbypassvalves.Following thereactortrip,theoperators utilizedtheEmergency Operating Procedures (E-0"ReactorTriporSafetyInjection<<

andES-0.1"ReactorTripResponse"

)tostabilize theplantinHotStandby(Operating Mode3).APPARENTCAUSEOFTHEEVENTThecauseforthiseventwasattributed tothefailureofthe<<C<<Main.Feedwater Regulating Valve(2FHS*FCV498).

ThisvalveisaCopes-Vulcan, ModelNo.D-100-160, PartNo.16-FA37RG.

whichaddressed thesafetysignificance ofthepotential loosepart(anti-rotation pinfrom2FWS*FCV498) whichcouldbetrappedinthe"C"steamgenerator tubesheet annulus.Itwasconcluded thattheestimated timerequiredforthelooseobjecttowearintoatubetotheminimumallowable wallthickness, is2.4years.TheNSSSconcurred withthissafetyevaluation justifying continued operation oftheremaining operating cycle.REPORTABI LITYThiseventwasreportedtotheNuclearRegulatory Commission at0300hoursinaccordance with10CFR50.72.b.2.ii.
