Event:

On September 26,1986, at 0035, and on October 6,1986, at 0457, blowdown isolation valves failed closed. Technical Specifications (TS) require the isolation of the affected penetrations within four hours. Power to the failed valves was isolated at 0605 on September 26, 1986, and at 1030 on October 6,1986, respectively, thus not meeting the TS requirements.

Cause of Event:

The cause of the event was personnel error in that the 4 hour4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> time limit to isolate the penetration was not met. The root cause for the failure of these valves was water from a leaking feedwater valve.

Corrective Action:

1) The personnel involved were counseled to exercise greater care in ensuring compliance with the applicable Technical Specifications.
2) The failed valves were repaired or replaced.
3) The leaking valve is being repaired.

On September 26,1986, at 0035, a blowdown isolation bypass valve (valve SV-4-62758-1) failed closed. Technical Specifications (TS) 3.3.3.b and 3.3.3.c require the isolation of any penetration which receives the Containment Phase A, Phase B, or Containment Ventilation isolation signal if the valve is inoperable. This isolation is to be accomplished within four (4) hours after the volve is found inoperable. Power to the failed valve was isolated at 0605 on September 26, thus exceeding the 4 hour4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> limit by I hour and 30 minutes. On October 6,1986, at 0457, blowdown isolation valve CV-4-6275C and blowdown isolation bypass valve SV 6275C-l failed closed. Power to the failed valve was isolated at 1030 on October 6, thus exceeding the 4 hour4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> limit by I hour and 33 minutes. The valves were repaired or replaced and returned to service at 1045 on September 26, and at 1614 on October 6, respectively.

Cause of Event:

The cause of the event was personnel error in that the 4 hour4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> time limit to isolate the penetration was not met. The cause of the failure of the above volves was water shorting out the solenoid. . The source of the water was a leaking valve in one of the Steam Generator Feedwater Pumps' suction line (valve 4-20-113).

Analysis of Event:

During the event, the redundant isolation valves for the affected penetrations remained operable. Based on the above, the health and safety of the public were not affected.

Corrective Action:

1) The personnel involved were counseled to exercise greater care in ensuring compliance with the applicable Technical Specification requirements.
2) Valve SV-4-62758-1 was replaced.
3) Valve 4-20-113 is being repaired in order to eliminate the source of the water.
