NOTICE OF VIOLATION Consumers Power Company Docket No. 50-155 As a result of the inspection conducted on May 23-27, 1988, and in accordance with 10 CFR Part 2, Appendix C - General Statement of Policy and Procedure for NRC Enforcement Actions (1987), the following violation was identified:

10 CFR 50, Appendix B, Criterion V, as implemented by Big Rock Point's "Quality Assurance Program Description for Operational Nuclear Power Plants," as described in Section 5.0, requires that activities be prescribed by procedures or drawings appropriate to the circumstances and shall be accomplished with these procedures or drawings.

Contrary to the above, maintenance procedure MGP-14. "Disassembly and Assembly of SMA-0 through SMA-4 Limitorques," Revision 3 and prints used for safety-related valves were inappropriate to the circumstances in that:

1. The procedure and prints were used to perform maintenance on safety-related SPA 000 and 00 model operators; and
2. Instructions were not provided for safety-related activities involving disassembly of the limit switch and identification of the limit switch lubricant.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office with thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective actior, taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
