I NOTICE OF VIOLATION Consumers Power Company Docket No. 50-155 -

. Big Rock Point Nuclear Plant License No. DPR-6 i During an NRC inspection conducted from November 30,1996, through January 17, 1997, two examples of violations of NRC requirements were identified. In accordance

with the " General Statement of Policy and Procedure for NRC Enforcement Actions,"

l NUREG-1600, the violation is listed below:

1. 10 CFR 50, Appendix B, Criterion V, " Instructions, Procedures, and Drawings,"

requires, in part, that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances.

Contrary to the above:

! A. On January 2,1997, surveillance procedure TV-02, " Containment Integrated 4

Leak Rate Test," Rev 28., an activite affecting quality, was inappropriate to j the circumstances in that performt .,e of the procedure resulted in 1) an i uncontrolled leak of about 400 gallons of reactor coolant through the open

CRD accumulator water-side-drain valves and 2} an inline relief valve lifting j due to the failure to remove a installed blank flange. j i

B. In November 1994, the work control documents used to inspect and repair l the main steam isolation valve, MO-7050, an activity affecting quality, were )

? inappropriate to the circumstances in that they did not contain specifications i to ensure a proper fit between the T-slot in the valve wedge and tee on the

- valve stem.

This is a Severity Level IV violation (Supplement 1). l
Pursuant to the provisions of 10 CFR 2.201, Consumers Power Company is hereby
required to submit a written statement or explanation to the U.S. Nuclear Regulatory i Commission, ATTN
Document Control Desk, Washington, D.C. 20555, with a copy to the Regional Administrator, Region lil, and a copy to the NRC Resident inspector at the i facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a ,

" Reply to a Notice of Violation" and should include for each violation: (1) the reason for '

t the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your

,l response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response. If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may

be issued as to why the license should not be modified, suspended, or revoked, or why

{ such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
