Preliminary License Application Waiver The purpose of this letter is to provide confirmation that Shane Marik, Phillip Warlow, and Richard Calvin will be enrolled in continuous licensed operator requalification training program. This is contingent upon successful completion of their NRC license exam schedu. led the week of April 21,1997.

Enrollment in this program will follow completion of the High Intensity Training program, currently scheduled to be completed by August 15,1997. 1 LaSalle County will provide written confirmation following completion of the j five reactivity manipulations required by each individual for issuance of their j license.
