l NOTICE OF VIOLATION

j Dear Mr. Feigenbaum:

On September 30,1996, the NRC completed an inspection at Seabrook Station. The

! enclosed report presents the results of that inspection.

.

During the six week period covered by this inspection period, overall, both management

and the staff conducted activities with an emphasis on safety. However, the station's

( planning and execution, as well as, the management eversight of the combustible gas

! control system maintenance was not completely effective, particularly when difficulties

l ' were experienced during the hydrogen analyzer celicalibration. It appears that the

~

!

involved technicians were not aware of the accuracy limitations of the analyzer system and

! had established acceptance and stabilization criteria for the cell calibration that could not

l be achieved. Resolution of these issues essentially consumed the seven-day allowed

' outage time defined by the associated Technical Specification action statement. The
. station director's decision to initiate an adverse condition report and perform a root cause

l

analysis, since the activity did not meet his expectations, demonstrated a good self-critical

- perspective.

During this inspection, certain of your activities appeared to be in violation of NRC

regulations. Specifically, the inspectors identified that floor drains located in the

emergency feedwater (EFW) pump house had been covered and isolated by tape during the

i . performance of general area and system component painting activities. The isolation of the

d

floor drains adversely impacted area flooding assumptions for rooms that contain safety-

related equipment. - The floor drains are credited in the' equipment qualification program to

mitigate the consequences of a moderate energy line break. The controls established to

prevent painting activities from impacting safety-related equipment were inadequate.

i Consequently, covering of the EFW pump house floor drains constituted a change to the j

i facility as described in the Updated Final Safety Analysis Report without the required 10  !

j; CFR 50.59 safety evaluation. )
