Waterford SES Unit No. 3 Docket No. 50-382 l Potentially Reportable Deficiency No. 171

" Failure of Emergency Diesel Generator (EDG) "A" to Start on Initiation of ESFAS" Final Report 1

1 On May 30, 1984, a failure of the EDG "A" to start on initiation of an Emergency Safety Features Actuation Signal (ESFAS) was reported as PRD No.

171. Further evaluation of the previously described condition indicates this condition is not considered reportable pursuant to 10CFR50.55(e).

Evaluation Troubleshooting indicated that the cable leads connected to one of the two Emergency Start circuits were landed on plastic screws. A detailed investigation was conducted into work and testing, which had been performed, on the EDG in the past eight months. It is felt that the plastic screws were installed during troubleshooting conducted during the period May 3-28, 1984.

Although, one signal was rendered inoperative due to the use of plastic screws an independent circuit would have caused EDG "A" to start and operate, if a valid ESFAS was present. In addition EDG "B" would have been available.

Mr. John T. Collins June 29, 1984 W3K84-1507 Page 2 The plastic screws were replaced with metal screws and EDG "A" successfully started utilizing an ESFAS signal. Plant personnel have received instruction concerning the use of procedure UNT-5-004, " Wire Removal and Jumper Control."

Based on the above evaluation LP&L considers this condition is not reportable pursuant to 10CFR50.55(e).
