Attachment 1 Georgia Institute of Technology Reply to Notice of Violation Inspection Report No. 50-160/94-02 Violation 50/16b/94-02, Severity Level IV (Supplement IV) 10 CFR 20.1501 requires that each licensee make or cause to be made surveys that may be necessary for the licensee to comply with the regulations in Part 20 and that are reasonable under the circumstances to evaluate the extent of radiation levels, concentrations or quantities of radioactive materials, and the potential radiological hazards that could be present.

Pursuant to 10 CFR 20.1003, survey means an evaluation of the radiological conditions and potential hazards incident to the production, use, transfer, release, disposal, or presence of radioactive material or other sources of radiation.

Contrary to the above, the licensee failed to make a proper evaluation of the extent of neutron radiation present following a survey performed on August 11, 1994, in that the conversion factor was inappropriately applied to the survey results which resulted in recorded dose rates that were much lower than the actual dose rates present in the area, by a factor of approximately 100.

Reply

1. Admission or Denial of Violation j The Georgia Institute of Technology agrees that a conversion factor was inappropriately applied to survey results on one occasion only (8/11/94). The error was not detected even though three different people reviewed the results.
2. The Reason for the Violation:

l The technician who was converting the neutron count rate per unit time to dose rate in mrem /hr erroneously divided by instead of multiplied by the conversion factor. The technician was relying on his memory of the procedure and consequently committed the error.

3. Corrective Steps Which Have Been Taken and the Results Achieved
a. The error was corrected on 8/16/94.

I b. A training session on document review was completed October 10, 1994.

c. The survey form on which the data were collected in counts per minute and converted to mrem / hour was modified to include the formula for converting CPM to mrem /hr.
