SIGNIFICANT CONSTRUCTION DEFICIENCY NO. 46 COOPER-BESSEMER EMERGENCY DIESEL GENERATORS AGASTAT RELAY DEFECT INTRODUCTION This report is submitted pursuant to 10CFR50.55(e). It describes a defect with the Agastat GPDC-740 control relays. The problem includes alarm failures and inability to effect emergency start. This problem is considered reportable under the requirements of 10CFR50.55(e). To the best of our knowledge, this problem has not been identified to the Nuclear Regulatory Commission pursuant to 10CFR21.

DESCRIPTION The Susquehanna licensee reported that preoperational testing of the diesels had identified problems with Agastat GPDC-740 control relays. Use of these relays was prescribed by the diesel manufacturer, Cooper-Bessemer. The problem included alarm failures and inability to effect emergeng start. Follow up on testing at Susquehanna of 156 relays found 82 failures.

The relay manufacturer indicates that the problem, post-mold plastic shrinkage, is applicable to all Agastat GP series relays manufactured before August 1977 (when the mold was changed).

SAFETY IMPLICATIONS It has been determined that the subject relay malfunction could result in a failure to effect the Diesel Generator start, if left uncorrected.

f The unavailability of one or both emergency power sources upon demand would be in conflict with GDC17 of 10CFR50 Appendix A.

CORRECTIVE ACTION 148 GFDC-740 control relays were removed and returned to Amarace Corporation for repair / replacement. In order to have minimal impact on startup testing, the de-fective relays associated with Diesel Generator A were returned first (74). -After repair, they were reinstalled and Diesel Generator B telays were returned for repair / replacement and subsequent reinstallation (74). Site Nonconformance Report W3-3534 was initiated to track this problem, and has been reviewed and closed, along with all Documentation subject to SCD 46. Therefore, we are submitting this report as our Final Report tc the USNRC.
