Dear Mr. O'Reilly:

The inspection report transmitted by Mr. Murphy's letter of November 28, 1978, has been reviewed by all concerned parties and we do not consider any of the inforr.ation contained therein to be proprietary.

We have taken the following action in response to the items noted in the inspection report:

4. Independent Inspection (Details II)

Infraction 364/78-17-02 Improper Storage of Electrical Penetration Assemblies (EPA)

(1) All electrical penetration assemblies have been cleaned. After cleaning, if work in progress did not require access to the internals, each penetra-tion assembly was closed.

(2) Field personnel have been instructed to keep pene-tration assemblies closed except when work requires access to the internals.

(3) Full compliance has been achieved as of December 15, 1978.

5. Instrumentation (Components and systems I) Observation of Work and Work Activities (Details II)

Infraction 364/78-17-01 Failure to Follow Procedure For Instrument Installation (1) The subject instruments have been reworked to comply with the requirements of the erection traveler.

(2) Piping Superintendents have been directed to be more attentive to the quality of craft workmanship.

Instrument installation inspectors have been reinstructed regarding their responsibility to verify compliance with procedures, drawings and specifications. Also, a drawing reading review has been conducted with emphasis on charac-teristics, symbols and notes.

(3) Full compliance has been achieved as of December 15, 1978.
