and J.J. Colitz (Tf!I-Unit 1 Superintendent) at 1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br />, Octcber 31,1974.

The following report is submitted in accordance with paragrcph 6.7.2.a.4 (page 6-11) of the technical specifications.

During an inspection of the reactor building on October 30, 1974, hydrculic shock suppressor, nucer Pb50, located on the pressurizar electron; tic relief valvo discharge line, et:s found errpty of hydraulic fluid.

The r:ain cylinder le:k nut was loose which parmitted the cylinder to rotste approxirately 900, hit another pipa which caused a connection betucen the reservoir and the lydraulic cylinder to break and drain the hydraulic fluid.

Tne broken fitting t as replaced, the hydraulic fluid was replaced, the suppecs-sor reoriented and the lock nut tightened.

The failed suppressor has a 2V' cylinder with a 5" piston stroke which was ranufactunid by Grinnel.
