RE: RII:CMH 50-348/79-42 This letter refers to the apparent infraction in the subject inspec-tion report which states:

"As required by Technical Specification 6.8.1, written procedures shall be established, implemented and maintained. Plant Procedure FNP-1-AP-52, Equipment Status Control and Maintenance Authorization, states that the releasing individual will review item 12 through 20 on the work request and will sign the " release for work" section only if the request is complete and accurate.

Contrary to the above, FNP Work Request 19749, which called for the repair of waste gas tank no. 8 inlet isolation valve (Q10-22V085B),

in accordance with Plant Procedure FNP-0-MP-21.1, was released for work although the plant conditions necessary for performing the work were not established. The Tagging Operations Order (No. 79-1671) referenced on the work request did not establish the plant conditions necessary to perform the work specified on the work request."

RESPONSE

Maintenance Work Request 19749 called for disassembly and repair of valve Q1G22V085B in accordance with FNP-0-MP-21.1. Subsequent to planning of the work request, operations informed the maintenance planners that the No. 8 Waste Gas Tank was pressurized and the valve could not be isolated from the tank. It was agreed to change the scope of the work to call for removal of the reach rod on the valve and to stroke the valve rather than to disassemble it. A Tagging Operations Order was executed to isolate the tank and valve from the remainder of the waste gas system. The Tagging Operations Order stated that the purpose of the clearance was to " stroke #8 GDT inlet valve".

.Mr. J. Philip Stohr Page 2 February 21, 1980 Work request 19749 was released by the Shift Foreman with the intent.that the valve was to be stroked. The work request instructions had not been revised, however, and the mechanic proceeded to start valve disassembly resulting in a small inadvertent gas release.

The Control Room was notified and the 121 ft., 100 ft., and 83 ft, elevations of the Auxiliary Building were cleared of personnel as a precautionary measure and the release was terminated.

Gas and . particulate samples taken in the immediate area of the' calve were negative. Personnel access to all elevations of the Auxiliary bailding was then re-established.

The following corrective action was completed by February 8, 1980.

'l. The incident was discussed with personnel involved to emphasize the need to have the work request reflect the actual work sequence that is to be accomplished and plant conditions which are required. Also, it was emphasized that conditions may require revision to the planning instructions during the planning phase.

2. This event has been used t.o emphasize the absolute necessity that the releasing individual u tisfy himself that plant conditions are satis-

' factory for release of the system. In conjunction with this, FNP-1-AP-52, Equipment Status Control and Maintenance Authorization, and l FNP-0-AP-14, Satety C1.earance and Tagging, were re-emphasized to all i licensed personnel durin'g requalification training.

This incident was previously reported to the NRC by phone and a letter was telexed on October 8, 1979; also a Non-Routine Environmental Operating Report was subsequently submitted.
