Page 1 of 1 Docket No. 50-338 Report No. LER 80-041/03L-0 Description of Event On May 3, 1980, with the unit in mode 1, it was discovered that the quarterly inservice inspection of the "B" LHSI pump check valves (1-SI-18, 1-SI-21,1-SI-26 and 1-SI-29) had not been performed within the required surveillance interval. The surveillance interval expired on April 21, 1980 and the descrepancy was discovered on May 3, 1980 at which time the required testing was satisfactorily performed.

Probable Consequences of Occurrence The consequences of this event were minimal because the testing was out of surveillance a total of only 12 days beyond the permitted Tech Spec. limit for a quarterly PT. Immediately upon discovery of the event, a satisfactory test was performed. As a result, the health and safety of the general public were not affected. There are no generic implications associated with this event.

Cause of Occurrence An error was made on the monthly Periodic Test Schedule from the Operation Department for the month of April, 1980, which inadvertantly omitted the subject test and it was not caught in subsequent reviews.

Immediate Correction Action The subject periodic test was immediately performed successfully.

Scheduled Corrective Action Periodic test schedules submitted by all applicable departments of the station will be reviewed promptly by the Performance Engineer for correctness and completeness.

Actions Taken to Prevent Recurrence Currently the station is in the process of developing a computer program to monitor the periodicity and completion dates of all station periodic tests. This system will aid in detecting any tests which are late cr have been inadvertantly omitted.
