LaSalle Operating Surveillance LDS-SC41 "28 $8LC Pop Operability /In Service Test and Explosive Continuity

  • Check" (required by Technical Specification 4.1.5) was past its critical (required) date (last performed on June 26, 1989; due (scheduled) Septentier 29, 1989; critical on October 22, 1989). The corresponding Unit 1 surveillance was determined to be siellarly affected, but was not required since the reactor was defueled I.

(last performed on June 10, 1989; due September 10, 1989; critical October 3,1989).

Upon discovery, the '8" $8LC was declared inoperable and a 7-day timeclock was commenced. LOS-$C41 Attactonent 8 was performed, evaluated as acceptable, and "B" $8LC declared operable at 2045 hours0.0237 days <br />0.568 hours <br />0.00338 weeks <br />7.781225e-4 months <br /> October 30,

The root cause of the event was a clerical data entry error which caused the conputerized scheduling program, General Surveillance (GSRV), software to suppress the surveillance from any normally published schedule.

This event is reportable to the NRC pursuant to the regulraments of 10CFR50.73(a)(2)(l) due to a condition prohibited by the plant Technical Specifications.

C. APPARENT CAUSE OF EVENT The root cause of the event was a clerical data entry error which caused the co g uterized scheduling s progra., General Surveillance (05RV), softwam to suppress the surveillance from any nomally pubilshed L schedule.

! I l A 05RV Change pequest was initlated to take the surveillance off an " Increased Frequency" status (due to

- pop vibration being in the alert range), properly approved, and submitted to Word Processing on approalmately July 14, 1989 to be incorporated into G5RV. Apparently, the Scope Operator entered a "0" into both the " Increased Frequency" fleid'and the " Critical Count" fleid rather than ,just the " Critical

' Count" fleid (i.e. the " Increased Frequency" field should have been blank". As normal, the Change Request form and resultant output reports were returned to the Station G5RV Coordinator the following

-Tuesday for validation. It was fou'wl that all changes were made as apparently requested, with no I corrections needed, and the normal schedules were issued to the Departments on the same day. It was not

,

noticed that the Scope Operator had made a change beyond that requested by the Change Request fom.

The "0" entry capabl11ty in the " Increased frequency" fleid was designed specifically for this purpose to be used at other facilities, but was elected not to be used at LaSalle Station. LaSalle l i

Aeninistrative Procedure LAP-100-11 "LaSalle County Station General Surveillanle Program" does not address the consequence of using "0" in this fleid, j

in accordance with LAP-100-11, two monthly supervisory reports are produced. 1he first is an independent schedule of all past due surveillances primarily intended to monitor the pool of overdue ,

surveillances. It too mitted the past due item for the same reason as the Departmental Schedules; i this, no revlower was able to identify the over due item. The second report, a list (as opposed to a I schedule) of over due items, very slallar in appearance, was intended to provide work planning detalls

of the over due items rather than a schedule to be executed by the assigned department.

Both reports were incorrectly assumed to include the same listing of overdue items; however, the list was generated by software which was not affected by the "0" this did contain the overdue item but was 1 revlowed for work detall and not for work execution.

Additional contributing factors to the eventi

a. The GSRV user's Manual does not address the potential effects of using a "0" in the " Increased frequency" field, b.- The Station GSRV Coordinator did not back check (verify that all changes made are the same as those requested) the transaction report from Work Processing to the original Change Request.  ;

D. SAFETY ANALYSIS OF EVENT ,

Given that 'the event involved only administrative issues and that the lamediate action to perform the i

subjectsurveillanceverlflodoperabilityofthesystem,thesafetysignificanceofthiseventis l minimal.

A related monthly survalliance, LOS-SC-MI "$8LC Pump Operability Test and Explosive valve continuity I' check", had been previously performed on October 23, 1989. The only significant difference between +

l LOS-SC41 and LOS-SC 41 is the inclusion of in Service Testing (pap vibration) measurements in the latter.

E. CORRECilVE ACTIONS

a. The missed survelliance was successfully performed, b.- All active surveillance records were searched for stellar errors. The same mistake was noted for Unit 1 and both were corrected.
