Docket No. 50-334 During normal startup operations, a steam line low pressure bistable [BS-MS-485]

began to intermittently trip and reset. The Nuclear Shift Supervisor (NSS) notified the Station Operating Supervisor (SOS) of the problem. The plant was in startup Mode 2 so the SOS went to the control room to be of assistance. The SOS asked the Nuclear Shif t Foreman what channel was alarming and was informed that it was channel [P-MS-484]. The SOS then contacted the control room from the process racks to verify that the status light came on when the bistable was tripped. When communication was noted, the SOS placed channel [P-MS-484] in trip. Since [P-MS-485] was the channel that was tripping, the logic made up (2/3) for a low steam line pressure safety injection.

The personnel involved in this incident were reprimanded and re-instructed via this LER and verbally of the importance of re-checking the steps necessary to complete an important evolution.
