  • UNIT NAME Point Beach Unit 1 DATE January 4, 1979 COMPLETED BY Wisconsin Electric Power Company Unit 1 was base loaded for approximately 94% of the period with a turbine runback and nine load reductions.

The turbine runback, which dropped load to 395 MWe, occurred at 0712 hours0.00824 days <br />0.198 hours <br />0.00118 weeks <br />2.70916e-4 months <br /> on December 19, 1978, and was caused by the failure of nuclear flux power range Channel N-44.

The channel was placed in the trip condition to restore the required minimum degree of redundancy. Return to full power commenced at 0722 hours0.00836 days <br />0.201 hours <br />0.00119 weeks <br />2.74721e-4 months <br /> and full power was achieved at 0738 hours0.00854 days <br />0.205 hours <br />0.00122 weeks <br />2.80809e-4 months <br />.

The runback caused a brief drop in reactor coolant system pressure to 2193 psia during the transient. According to the Technical Specifications, pressure may not drop below 2220 psia when normal operating pressure is 2250 psia. This, along with the brief period of operation with a reduced de.yree of redundancy, required a 14-day licensee event report t9 the NRC (Licensee Event Report No. 78-018/0lT-0). The cause of the channel failure was determined to be due to problems in the high voltage power supply. A spare power supply was installed and the channel was returned to service on December 22, 1978.

Load was reduced to 250 MWe for nine hours December 2, 1978, to locate and plug a small condenser tube leak in the No. 4 waterbox. During the load reduction, control rod K-7 became misaligned with its bank resulting in a quadrant flux tilt. Licensee Event Report No. 78-017/0lT-0 provided a description of the circumstances in which the problem occurred.

Power Supply requested on eight occasions that load be reduced to an average of 355 MWe for an average duration of four hours each.

A 14-day reportable event (Licensee Event Report No.

78-019/0lT-0) occurred on December 28, 1978, with the discovery of the failure of both heat tracing circuits for a section of boric acid piping between the "B" boric acid pump discharge valve and the boric acid filter. Investigation disclosed defective potentiometers in each circuit which caused voltage spikes and resulted in the circuit fuses burning out. Both potentiometers were replaced and both circuits were returned to service on December 28, 1978. Temperature

2 recordings indicate that the section of line stayed above the 1450F minimum temperature permitted by the Technical Specifications despite the loss of heat tracing.

No other major safety-related maintenance was performed during the period.

The outage ended a 233 day run of continuous on-line operation.

The unit was taken off line for 25 hours2.893519e-4 days <br />0.00694 hours <br />4.133598e-5 weeks <br />9.5125e-6 months <br /> December 9-10, 1978, to repair moisture separator reheater (MSR) tube leaks.

Load reduction for the outage commenced at 11:34 p.m.,

December 8, 1978, and the unit was taken off line at 3:23 ,

a.m., December 9, 1978. Eight tubes were plugged in the "A" moisture separator reheater, five in the "B", nine in the "C" and nine in the "D". Condenser tube leak checks and turbine stop valve tests were conducted during the outage.

Moisture separator reheater repairs were completed at 11:00 p,m., December 9, 1978. The unit was placed on line at 4:24 a.m., December 10, 1978, and full load was achieved at 8:55 a.m., December 10, 1978.

A 14-day reportable occurrence (Licensee Event Rcport No. 78-012/0lT-0) took place during the Unit 2 moisture separator reheater outage December 9, 1978, when the react o--

coolant system "A" loop resistance temperature detector ( R'.?D )

bypass manifold was manually icolated with the reactor just critical. This violated the Technical Specifications since the requirements for the minimum number of operable channels and the minimum degree of redundancy for the overtemperature AT and overpower AT trips were not met.

Load was reduced to 490 MWe for two and one-half hours early on December 14, 1978, to check for tube leakage in the 5B feedwater heater. Leakage was detected and a maintenance request was issued.

About two and one-half hours later the 5B feedwater heater dump valve stuck open and caused a drop ir steam generator feed pump suction pressure. A manual runback to '

455 MWe for 20 minutes was required to prevent r. unit trip and permit the valve to be manually closed before full power operation could Le resumed.

Late December 14, 1978, load was :: educed to 445 MWe for three b;urs to isolate the 5B feedwater heater for repairs.

After tie heater was isolated, the unit was returned to its full capacity of 490 MWe in that condition. The unit operated at 490 MWe for 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> while the leaking tubes were plugged.
