j The inspection was an examination of the activities conducted under your  l license as they relate to radiation safety and to compliance with the Commission's rules and regulations and the conditions of your license. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspecto ;
Based on the results of this inspection, it appears that certain of your  !
activities were not conducted in full compliance with NRC requirements, as set l forth in the Notice of Violation enclosed as Appendix A to this lette These l items have been categorized into severity levels as described in the NRC  j Enforcement Policy, 10 CFR Part 2, Appendix C (1989). i

l The findings from this inspection, as well as the one conducted during August 27, 1984, indicate two repetitive violations (see Appendix A, Items B. and C. ).
We are concerned about the repetitive nature of the violations observed during our inspections. In addition to the need for corrective action regarding the i specific violations included in Appendix A, we are especially concerned about j the effectiveness of your management control system that permitted the  j violations to recur. Consequently, in your reply you should describe in  l particular those actions taken or planned to improve the effectiveness of your j overall management control syste On September 6, 1989, Messrs. R. D. Thomas and D. D. Skov held a telephone j management meeting with you regarding the findings of the inspection. During ;
this meeting, we emphasized the seriousness of the repetitive violations and we discussed our concerns with your management of the licensed program. You ;
