Description:
Sequence of Events This information was collected from the Hathaway recorder and chart recorders IC34-R608 (Upset Range Level & Narrow Range Level Div I),
1B21-R884A (Wide Range Pressure and-Wide Range Level Div. I) and 1B21-R884B (Wide Range Pressure and Wide Range Level Div. II)'
Also, the operator's log.
Dec. 19, 1982 1100 Operators started to lower water level for U1 S/U 1215 Water level reached 34" 1230 Mode switch was put in S/U 1350 Water level exceeded 40", causing a high level alarm. Water level-remained high because of NPSH considerations on the RWCU pumps.
1420-1450 Water level reached 47" and was blown down to 44" using the RWCU System. At this time Rx pressure indicated zero. U1 went critical. Div. II narrow range (B) was noted by the operator to be on scale and shift change occurred.
1450-1610 Water level increased to 58". Rx. pressure still at zero. Rx water level was raised due to NPSH problems on the RWCU pumps (previously noted). Rx was in Heatup phase.
1610-1810 Reactor water level remained between 57"-60" and Rx pressure increased from 0-80#'s during this time frame. Throughout this period, Div. I and Div. II wide range level recorders indicated levels greater than 60".
1810-1900 At 1818 hours0.021 days <br />0.505 hours <br />0.00301 weeks <br />6.91749e-4 months <br /> the Div. II channel B (Rx Vessel Hi Wtr Level Trip)
Reset as shown on the Hathaway printout. This time appears to correspond with a decrease in Rx level below 40" on recorder 1C34-R608 (Narrow Range Level Div. I). Rx pressure was at 130#'s. Level was decreased from 57" to 34" with RWCU blow down.
Towards the end of this time period Rx Level began to increase due to Heatup and continuous CRD inputs.
1900-2030 Rx Level rose from about 38" to 47" due to Heatup & CRD inputs. At 2009 hours0.0233 days <br />0.558 hours <br />0.00332 weeks <br />7.644245e-4 months <br /> the Div. II channel B (Rx Vessel Hi Water Level Trip) Reset per the Hathaway Recorder. Around this time frame, a decrease in Rx Vessel Level below 40" was experienced. Rx pressure increased from 130-300 psi. Both wide range recorders were still indicating high level.
2030-2200 Rx Level stabilized at about 36" and Rx pressure increased from 300-600 psi. According to the Hathaway printout, at 2112 the Div. II channel B (Rx Vessel Hi Water Level Trip) tripped. The Chart Recorder IC34-R608 reading shows that a Rx Vessel Level of greater than 40" n.m - - - , - n n m a ~,
occurred at approximately this time. "At 2200 wide range level A came on scale and read 42". B wide range recorder (1B21-R884B) still read high.
2200-00:00 Rx level remained at 36" and Rx pressure increased from approxi-mately 600-800 psi. ('A') Div I wide range indicated Rx level stabilized at 38". ('B') Div II wide range recorder remained high. The afternoon shift suspected that there may have been a vessel instrumentation problem and instructed an Instrument Mainte-nance technician to go to the local instrument racks and check out a few instruments to look for leaks etc. (This happened between 2215-2300 hours). The night shift was informed that there may be a vessel level instrumentation problem. Shift change occurred. The s IM was contacted between 2300-0000 and told to pursue the matter further and that the problem may deal with a Rx vessel instrumenta -

tion reference leg. At 2307 an alarm summary was printed out on the Hathaway which showed a Div II Channel B1 (Rx vessel High Water Level Trip) tripped. The alarm did not clear after Rx Vessel Level '
went below 40" at 2120 hours0.0245 days <br />0.589 hours <br />0.00351 weeks <br />8.0666e-4 months <br /> as indicated on 1C34-R608. At 00:00 it was determined that Div II level instrumentation was inoperative.
0000-0200 Rx level remained normal as shown by Div I level. Rx pressure was at 850 psi throughout this time frame. The NRC was notified about the occurrence. At 0100 a manual 1/2 scram was taken on channel Bl.
At 0105 commenced procedure to go to Hot Shutdown. During this time period, the IM refilled the Div II level instrumentation reference leg satisfactorily once the leaky fitting on an instrument line to LTIS 1B21-N044B was tightened. From the Hathaway printout Div II level instrumentation was restored at 0109 and the B wide range recorder came on scale and read 38". The 'B' High Rx level channel reset at 0140 (from Hathaway). The channel B11/2 scram was reset.
At 0200 startup was resumed and Div II level instrumentation was declared operable.

V. PROBABLE CONSEQUENCES OF THE OCCURRENCE:
At the time of the occurrence, LSCS was in the startup mode at 33MWT. The reference leg problem resulted in a loss of Division II instrumentation for the B and C RHR pumps, lA Diesel Generator, "B" logic of ADS and PCIS Groups 1-7.
Division I vessel instrumentation was providing sufficiant indication of reactor temperature, water level and pressure to maintain the plant at proper operating conditions. Division III was operable and available throughout this event. A scram would have been able to occur during this happening because 3 of 4 RPS channels were operable. When the problem was recognized, appropriate and timely corrective action was taken. Safe operation of the unit was r.aintained at all times.

VI. CAUSE:
The cause of the occurrence was found to be a leaky fitting on line INB56A " at'LTIS 1B21-N044B which is located on panel 1H22-P009. This fitting was used as a tap for a hydro after recent construction work while the plant was shutdown. No provisions were made to verify that th'e ,
fitting held pressure during the ensuing startup. and plant pressurization, and as reactor pressure increased above 300 psi the reference leg then began to leak past the fitting.
Problems brought to light in this event include: 1) The need for some additi.aal guidance for the plant operatore when instrumentation failures involving multiple reactor vessel parameters are involved. Existing procedures do not appear to be adequate. 2) This event indicated the need to verify that the fitting which was broken for the hydro rig was reconnected properly. This couldn't be done until the subsequent plant heat up.
VII. CORRECTIVE ACTION: ,
An Instrument Maintenance technician assisted by various operating personnel began investigating a possible instrument problem shortly after 2200, 12/19/82.
At approximately 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> he was instructed to search for a possible reference leg problem. Subsequently, the leaky fitting on line 1NB56A "
was detected by the Instrument Maintenance technician at 0000, 12/20/82 and a manual 1/2 scram was taken in order to comply with Technical Specification 3.3.1 at 0100, 12/20/82. The fitting was tightened and the reference leg was backfilled. Division II level indication was declared operable at 0200.
An Action Item Request #01-83-19 has been written requesting that an evaluation be conducted to determine if a procedure should be developed to aid the operator in determining if there has been a common mode failure to Reactor Vessel level instrumentation.
An Action Item Request #01-83-20 has been written to require the General Startup Procedure to include a sign off to verify the Wide Range Level ,
detectors begin to come on scale at the appropriate pressure.
A letter has been issued by the Station Superintendent directing the required parties to ensure work requests that break mechanical fittings on instruments are verified by inspection to be properly sealed.
