p.. On October 2nd a tube break occurred in No. 11 Steam Generator. The resultant k leakage caused a reactor trip at 1424 hours0.0165 days <br />0.396 hours <br />0.00235 weeks <br />5.41832e-4 months <br /> on low pressurizer pressure and safety injection initiation five seconds later. All safety systems functioned as designed, and the reactor was d.. awn and cooled down and the steam gener-ator isolated expeditiously following existing operating procedures. It was found that a foreign object (a spring) had caused mechanical wear of the fail-ed tube and one or two more tubes. Six tubes were plugged, a pressure test conducted and the steam generator was returned to service. The unit was re-turned to service at 0645 on October 23rd.
