, On October 30,1989 at 1830 hours0.0212 days
0.508 hours
0.00303 weeks
6.96315e-4 months
, the on-4uty shif t Control Room Engineer (SCRE) reviewed a list of past due I technical specification related survelilances and noted that the quarterly Standby Llquid Control (58LC) (SR)
LaSalle Operating Surveillance LDS-SC41 "28 $8LC Pop Operability /In Service Test and Explosive Continuity
- Check" (required by Technical Specification 4.1.5) was past its critical (required) date (last performed on June 26, 1989; due (scheduled) Septentier 29, 1989; critical on October 22, 1989). The corresponding Unit 1 surveillance was determined to be siellarly affected, but was not required since the reactor was defueled I.
(last performed on June 10, 1989; due September 10, 1989; critical October 3,1989).
Upon discovery, the '8" $8LC was declared inoperable and a 7-day timeclock was commenced. LOS-$C41 Attactonent 8 was performed, evaluated as acceptable, and "B" $8LC declared operable at 2045 hours0.0237 days
0.568 hours
0.00338 weeks
7.781225e-4 months
October 30, 1989.
The root cause of the event was a clerical data entry error which caused the conputerized scheduling program, General Surveillance (GSRV), software to suppress the surveillance from any normally published schedule.
This event is reportable to the NRC pursuant to the regulraments of 10CFR50.73(a)(2)(l) due to a condition prohibited by the plant Technical Specifications.

On October 30, 1999 at 1830 hours0.0212 days
0.508 hours
0.00303 weeks
6.96315e-4 months
, the on duty Shift Control hoom Engineer (SCRE, licensed Senior l
,l heactor Operator) reviewed a list of past due technical specification related surveillances and noted ,
.that the quarterly Standby 1.lquid Control ($bLC) [BR) LaSalle operating Surveillance LOS-SC41 "25 SSLC ,
i pop Operability /In Service Test and Emplosive Continuity Check" (required by Technical Specifications y - 4.0.5 and 4.1.5) was currently past its critical (with allow 25 percent extension) date (last performed on June 26, 1989;. due (scheduled) Septauber 29, 1989; critical on October 22,1999). The corresponding .
Unit I surveillance was detemined to be slallarly affected, but was not required since the reactor was l defueled (last perfomed on June 10, 1999; due September 10, 1909;~ critical October 3, 1989). l L
L
.Upon discovery, the "B" SSLC system was declared inoperable and a 7 day time clock per Technica) >
l Specification 3.1.5 action a.1. was cemenced. LOS-SC41 Attachment 8 was perfomed, evaluated as acceptable, and "B" SSLC declared operable at 2045 hours0.0237 days
0.568 hours
0.00338 weeks
7.781225e-4 months
October 30, 1999.
i This event ls reportable to the NRC pursuant to the requirements of 10CFR50.73(a)(2)(1) due to a 5
! condition prohibited by the plant Technical Specifications.
f C. APPARENT CAUSE OF EVENT The root cause of the event was a clerical data entry error which caused the co g uterized scheduling s progra., General Surveillance (05RV), softwam to suppress the surveillance from any nomally pubilshed L schedule.
! I l A 05RV Change pequest was initlated to take the surveillance off an " Increased Frequency" status (due to
- pop vibration being in the alert range), properly approved, and submitted to Word Processing on approalmately July 14, 1989 to be incorporated into G5RV. Apparently, the Scope Operator entered a "0" into both the " Increased Frequency" fleid'and the " Critical Count" fleid rather than ,just the " Critical
' Count" fleid (i.e. the " Increased Frequency" field should have been blank". As normal, the Change Request form and resultant output reports were returned to the Station G5RV Coordinator the following
-Tuesday for validation. It was fou'wl that all changes were made as apparently requested, with no I corrections needed, and the normal schedules were issued to the Departments on the same day. It was not noticed that the Scope Operator had made a change beyond that requested by the Change Request fom.

C. APPARENT CAUSE OF EVENT (Continued)
I The "0" entry capabl11ty in the " Increased frequency" fleid was designed specifically for this purpose to be used at other facilities, but was elected not to be used at LaSalle Station. LaSalle l i
Aeninistrative Procedure LAP-100-11 "LaSalle County Station General Surveillanle Program" does not address the consequence of using "0" in this fleid, j in accordance with LAP-100-11, two monthly supervisory reports are produced. 1he first is an independent schedule of all past due surveillances primarily intended to monitor the pool of overdue ,
surveillances. It too mitted the past due item for the same reason as the Departmental Schedules; i this, no revlower was able to identify the over due item. The second report, a list (as opposed to a I schedule) of over due items, very slallar in appearance, was intended to provide work planning detalls of the over due items rather than a schedule to be executed by the assigned department.
Both reports were incorrectly assumed to include the same listing of overdue items; however, the list was generated by software which was not affected by the "0" this did contain the overdue item but was 1 revlowed for work detall and not for work execution.
Additional contributing factors to the eventi
- a. The GSRV user's Manual does not address the potential effects of using a "0" in the " Increased frequency" field, b.- The Station GSRV Coordinator did not back check (verify that all changes made are the same as those requested) the transaction report from Work Processing to the original Change Request. ;
1 D. SAFETY ANALYSIS OF EVENT ,
Given that 'the event involved only administrative issues and that the lamediate action to perform the i
subjectsurveillanceverlflodoperabilityofthesystem,thesafetysignificanceofthiseventis l minimal.
A related monthly survalliance, LOS-SC-MI "$8LC Pump Operability Test and Explosive valve continuity I' check", had been previously performed on October 23, 1989. The only significant difference between +
l LOS-SC41 and LOS-SC 41 is the inclusion of in Service Testing (pap vibration) measurements in the latter.
E. CORRECilVE ACTIONS
- a. The missed survelliance was successfully performed, b.- All active surveillance records were searched for stellar errors. The same mistake was noted for Unit 1 and both were corrected.

- 0. $AFETY ANALYSIS OF EVENT (Continued)
- c. Requests to modify 0$#V will be initiated to:
- 1. Re61 sed the user's Manual to address the potential effects of using a "0" in the "Increesed l
Frequency" fleid, and ?
- 2. Revise the " Transaction Report" (an electronically generated report returned to the 05RY i
Coordinator from Word Processing which Indicates all changes actually entered into the 05RV records) to more clearly indicate the changes entered, and
- 3. Revised the " file Error Report" (and electronically generated report from Word Processing l which lists all records containing data whict, violates logic statements written into the l software and not accepted into the data base) to include LaSalle Station records which contain a *0" in the " increased Frequency" fleid. :
Action item Record (AIR) 374-200-09-06101 will track completion of this request.
- d. ' The Overdue Surveillance Report issued to the Operating Department has been reformatted to more clearly identify overdue items, r
- e. The Station 05RV Coordinator has begun, and will continue, back check (verify that all changes made are the same as those requested) all data entry and correct any errors affecting due date and critical date calculations, and routinely examine the data base for "0" in the " Increased Frequency" fleid until the GSRV software is modified to reject the undesired "0" for LaSalle ,
L I.
Station. AIR 374-200-89-06102 will track the coupletion of this item.
j ..
- f. The Scope operators have been made aware of the possible effects of seemingly minor errors and the need for accuracy.
i
- g. LAP-100-11 will be revised to include a precaution regarding the use of "0" In the " Increased Frequency" fleid. AIR 374-200-89-06103 will track the conpletion of this ites.
