The resultant k leakage caused a reactor trip at 1424 hours on low pressurizer pressure and safety injection initiation five seconds later. All safety systems functioned as designed, and the reactor was d.. awn and cooled down and the steam gener-ator isolated expeditiously following existing operating procedures. It was found that a foreign object (a spring) had caused mechanical wear of the fail-ed tube and one or two more tubes. Six tubes were plugged, a pressure test conducted and the steam generator was returned to service. The unit was re-turned to service at 0645 on October 23rd." 9 122'7911140

p.. On October 2nd a tube break occurred in No. 11 Steam Generator. The resultant k leakage caused a reactor trip at 1424 hours0.0165 days <br />0.396 hours <br />0.00235 weeks <br />5.41832e-4 months <br /> on low pressurizer pressure and safety injection initiation five seconds later. All safety systems functioned as designed, and the reactor was d.. awn and cooled down and the steam gener-ator isolated expeditiously following existing operating procedures. It was found that a foreign object (a spring) had caused mechanical wear of the fail-ed tube and one or two more tubes. Six tubes were plugged, a pressure test conducted and the steam generator was returned to service. The unit was re-turned to service at 0645 on October 23rd.
